摘要
目的以回顾性分析形式论证口腔种植修复中附着龈重建的价值.方法纳入某院于2015年4月—2016年5月行附着龈重建术治疗的患者60例为该次研究对象,以入组时间的不同将60例患者平均分为研究组和对照组,每组30例.研究组行附着龈重建术治疗,对照组行常规治疗,对比两组治疗后的附着龈宽度以及龈缘外形和附着点重建存在缺陷的例数.结果研究组排除1例患者因治疗过程中牙周塞治剂脱落,造成手术14 d后黏膜依然存在炎性反应,其余患者黏膜伤口愈合位置显著,对照组治疗1个月后对患者佩戴牙冠过程中,依然有3例患者手术位置黏膜处存在炎性反应.手术后90 d研究组附着龈宽度(2.5±1.2)mm高于对照组(1.6±0.2)mm,手术后半年研究组附着龈宽度(3.1±0.6)mm高于对照组(2.6±0.8)mm,组间对比差异有统计学意义(t=4.0520,2.7386,P<0.05).研究组手术后90 d 1例缺陷,对照组6例缺陷,手术后半年研究组0例缺陷,对照组4例缺陷,两组对比差异有统计学意义(x^2=4.0431、4.2857 P<0.05).结论附着龈重建技术应用于口腔种植修复中,能够提升临床修复效果,对修复情况改善外,降低重建失败率.同时更进一步的利于患者口腔患侧位置修复后牙龈的稳定度,根据美学方面思考,附着龈重建手术的意义重大,同时此种手术方法简单可操作.
Objective To demonstrate the value of attachment gingival reconstruction in dental implant prosthesis by retrospective analysis. Methods A total of 60 patients who underwent attached gingival reconstruction in the Hospital from April 2015 to May 2016 were enrolled in this study. According to the different time of enrollment, 60 patients were divided into study group and control group, 30 cases in each group. The study group was treated with attachment gingival reconstruction, while the control group was treated with routine treatment. The width of attachment gingiva, the shape of gingival margin and the number of defective attachment reconstruction were compared between the two groups. Results The study group excluded one patient whose periodontal plug had fallen off during the treatment, which caused the mucosal inflammatory reaction 14 days after the operation. The other patients' mucosal wound healing position was significant. In the control group, there were still three patients whose mucosal inflammatory reaction occurred at the operation site during the wearing of crowns one month after the treatment. The width of attachment gingiva in the study group 90 d was (2.5±1.2) mm, which was higher than that in the control group (1.6±0.2) mm after operation. The width of attachment gingiva in the study group half a year after operation was (3.1±0.6) mm, which was higher than that in the control group (2.6±0.8) mm. There was statistical significance between the two groups (t=4.052 0,2.738 6, P<0.05). There was 1 defect in the study group 90 d after operation, and 6 defect in the control group. There was 0 defect in the study group 6 months after operation, and 4 defect in the control group. There was statistically significant difference between the two groups (χ^2=4.0431, 4.2857, P<0.05). Conclusion The application of attached gingival reconstruction technology in dental implant prosthesis can improve the clinical prosthetic effect, improve the prosthetic condition and reduce the failure rate of reconstruction. At the same time, it further benefits to the stability of the gingiva after restoration of the affected side of the patient's oral cavity. According to aesthetic considerations, attachment gingival reconstruction surgery is of great significance, which is simple and operable.
