128层螺旋CTA在可疑急性冠脉综合征诊断中的应用价值 被引量:5

摘要 目的:探讨128层螺旋CTA在可疑急性冠脉综合征诊断中的应用价值。方法:选择2017年8月-2018年12月在本院治疗的疑似急性冠脉综合征患者60例,均做CTA与冠状动脉造影(CAG)DSA检查,冠脉CTA采用冠状动脉追踪冻结技术(snap shot freeze,SSF)。以CAG检查为最终诊断结果,观察CTA检查诊断急性冠脉综合征的灵敏度、特异度,以及CTA、CAG两种检查方法诊断冠状动脉狭窄程度情况。结果:以CAG检查为标准,CTA诊断急性冠脉综合征的灵敏度为95.83%(46/48),特异度为91.60%(11/12)。CTA、CAG两种检查方法对于冠状动脉通畅、轻度狭窄、中度狭窄、重度狭窄、闭塞的检出率比较,差异均无统计学意义(P>0.05)。60例患者CTA评分1分42例,2分14例,3分3例,4分1例,平均(1.52±0.33)分;有效图像59例,占98.33%。结论:CTA在急性冠脉综合征诊断中具有重要的临床应用价值,能够及时、准确地为疾病的诊断和鉴别诊断提供重要依据,且具有方便、价廉的优势。 Objective:To investigate the value of 128-slice spiral CTA in the diagnosis of suspicious acute coronary syndrome.Method:Sixty patients with suspected acute coronary syndrome treated in our hospital from August 2017 to December 2018 were selected,CTA and CAG DSA were performed,coronary artery CTA adopts coronary artery tracing and freezing technique,the sensitivity and specificity of CTA in the diagnosis of acute coronary syndrome were observed,the degree of coronary artery stenosis was diagnosed by CTA,CAG.Result:Using CAG test as the standard,the sensitivity and specificity of CTA in the diagnosis of acute coronary syndrome were 95.83%(46/48)and 91.67%(11/12).There were no significant differences in the detection rates of coronary artery patency,mild stenosis,moderate stenosis,severe stenosis and occlusion between the CTA and CAG(P>0.05).CTA score of 60 patients was 1 point in 42 cases,2 points in 14 cases,3 points in 3 cases and4 points in 1 case,with an average score of(1.52±0.33),valid images were obtained in 59 cases(98.33%).Conclusion:CTA has an important clinical application value in the diagnosis of acute coronary syndrome,which can provide important basis for the diagnosis and differential diagnosis of diseases timely and accurately,and has the advantage of convenience and low price.
