摘要
目的:探讨冠心病相关慢性心力衰竭(CHF)患者血清趋化因子调节活化正常T细胞表达与分泌蛋白(RANTES)水平与心功能的相关性。方法:选取CHF患者80例,根据病因不同,将由冠心病引起的CHF患者48例作为观察组,非冠心病相关CHF患者32例为对照组;另将观察组根据心功能水平不同分为三个亚组,Ⅱ级12例、Ⅲ级22例、Ⅳ级14例。检测并比较两组及不同心功能分级者的RANTES、脑钠肽(BNP)及左室射血分数(LVEF)水平;分析冠心病相关CHF患者血清RANTES水平与心功能分级、BNP、LVEF的相关性。结果:观察组RANTES水平高于对照组(P<0.05);不同心功能分级患者RANTES、BNP、LVEF水平比较,差异均有统计学意义(P<0.05);Pearson相关分析显示,观察组血清RANTES水平与心功能分级、BNP均呈正相关(r=0.735、0.693,P<0.01),与LVEF呈负相关(r=-0.625,P<0.01)。结论:血清趋化因子RANTES水平与冠心病相关慢性心力衰竭患者心功能密切相关,可考虑作为评估冠心病相关CHF患者病情严重程度的指标。
Objective:To investigate the relationship between the levels of serum chemokine regulated upon activation normal T cell expressed and secreted(RANTES)and the cardiac function of coronary heart disease complicated with chronic heart failure(CHF).Method:A total of 80 patients with CHF were selected,according to the different etiology,48 CHF patients caused by coronary heart disease were taken as observation group,32 CHF patients not related to coronary heart disease were taken as control group,in addition,according to the different level of cardiac function,the observation group was divided into three subgroups:12 cases of grade Ⅱ,22 cases of grade Ⅲ and 14 cases of grade Ⅳ.The levels of RANTES,brain natriuretic peptide(BNP)and left ventricular ejection fraction(LVEF)were measured and compared between two groups and those with different cardiac function grades,the correlation between serum RANTES level and cardiac function classification,BNP and LVEF in CHF patients with coronary heart disease were analyzed.Result:The level of RANTES in observation group was higher than that of control group(P<0.05).The levels of RANTES,BNP and LVEF in patients with different cardiac function grades were compared,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that serum RANTES level were positively correlated with cardiac function grade and BNP(r=0.735,0.693,P<0.01)and negatively correlated with LVEF(r=-0.625,P<0.01).Conclusion:The level of serum chemokine RANTES is closely related to the cardiac function of patients with CHF associated with coronary heart disease,it can be considered as an index to evaluate the severity of CHF associated with coronary heart disease.
