Mulligan手法联合肩关节腔灌注治疗肩周炎效果观察

Therapeutic Effect of Mulligan Technique Combined with Shoulder Joint Cavity Perfusion on Frozen Shoulder

目的:探讨Mulligan动态关节松动术联合肩关节腔灌注治疗粘连性肩周炎患者疼痛及肩关节活功能改善的效果,为肩周炎患者治疗提供一种可选方法。方法:选择门诊或住院肩周炎患者50例,按照随机数字表法将其分为对照组和治疗组,每组25例。两组均采用威伐光、超短波治疗、热敷等常规理疗治疗。对照组以Mulligan动态关节松动手法治疗为主,治疗组以Mulligan关节松动手法+肩关节腔灌注治疗为主,一个疗程(10 d)结束时,采用目测类比评分法(visual analogue scale,VAS)、肩关节功能评定量表进行治疗前后评估对比,并统计治疗总有效率。结果:两组治疗后VAS评分、肩关节功能评分较治疗前均有明显改善(P<0.05),治疗组较对照组均明显改善(P<0.05)。治疗组总有效率为96%,与对照组的76%相比,差异有统计学意义(χ^2=2.04,P<0.05)。结论:Mulligan手法联合肩关节腔灌注在改善肩周炎患者疼痛及肩关节功能障方面效果显著,可明显缩短临床治疗疗程,经济效益及社会效益良好。

Objective:To investigate the effect of Mulligan technique combined with shoulder joint cavity perfusion in the treatment of shoulder and shoulder dysfunction in patients with adhesive shoulder scapulohumeral and the improvement of shoulder function,to provide an alternative method for the treatment of shoulder scapulohumeral.Method:Fifty patients with outpatient or hospitalized periarthritis were selected and divided into the control group and the treatment group according to digital random table method,25 cases in each group.Both groups were treated with conventional physiotherapy such as weivar,ultrashort wave therapy and hot compress.The control group was treated with Mulligan technique,the treatment group was treated with Mulligan joint loosening method+shoulder joint perfusion therapy.At the end of a course of treatment(10 d),visual analog scale(VAS)and shoulder joint were used.The functional rating scale was used to evaluate the comparison before and after treatment,and to measure the effectiveness of the treatment.Result:After treatment,the scores of VAS and shoulder function evaluation scales were significantly improved compared with those of before treatment(P<0.05),the experimental group were better than those in the control group(P<0.05).The total effective rate was 96% in the treatment group,which was higher than 76% in the control group(χ^2=2.04,P<0.05).Conclusion:Mulligan technique combined with shoulder joint perfusion is effective in improving the special pain and shoulder function of patients with frozen shoulder,which can significantly shorten the clinical treatment course and economy.Good benefits and social benefits are worthy of clinical application.
