基于5E模式的综合护理对腹膜透析患者血压控制、自护能力及社会功能的影响

Effect of Comprehensive Nursing Based on 5E Model on Blood Pressure Control,Self-care Ability and Social Function of Peritoneal Dialysis Patients

目的:探讨基于5E模式的综合护理对腹膜透析患者血压控制、自护能力及社会功能的影响。方法:选取2016年2月-2018年2月于本院接受诊治的腹膜透析患者140例,依据实施护理方案的不同进行分组,对照组70例患者实施常规护理,观察组70例患者实施基于5E模式的综合护理,记录两组血压控制情况、心血管并发症发生情况及住院次数,评定两组自护能力,采用KDQOL-SF36量表测评两组生活质量情况,观察社会功能变化情况。结果:护理后两组舒张压、收缩压均明显下降(P<0.05),血压达标率明显升高(P<0.05),且组间比较差异均有统计学意义(P<0.05);观察组心血管并发症发生情况与住院次数均明显优于对照组(P<0.05);观察组自护能力各维度指标评分均明显高于对照组(P<0.05);观察组SF-36量表各维度评分均明显优于对照组(P<0.05)。结论:腹膜透析患者实施基于5E模式的综合护理,在血压控制、降低心血管并发症发生、减少住院次数等方面成效显著,在增强患者自护能力、改善生活质量、强化社会功能等方面亦可起到积极推进作用。

Objective:To explore the effect of comprehensive nursing based on 5 E model on blood pressure control,self-care ability and social function of peritoneal dialysis patients.Method:A total of 140 peritoneal dialysis patients who received treatment in our hospital from February 2016 to February 2018 were randomly divided into the control group(routine nursing,n=70)and the observation group(comprehensive nursing based on 5 E model,n=70)based on nursing mode.Blood pressure control,cardiovascular complications and hospitalization times were recorded,self-care ability of two groups was evaluated,and quality of life of two groups was evaluated by KDQOL-SF36 scale,and changes in social function were observed.Result:Diastolic blood pressure and systolic blood pressure of two groups decreased significantly after nursing(P<0.05),and the rate of reaching the standard of blood pressure increased significantly(P<0.05),and there were statistically significant differences between two groups(P<0.05).The incidence of cardiovascular complications and the number of hospitalizations in the observation group were significantly better than those in the control group(P<0.05).The scores of each dimension of self-care ability in the observation group were significantly higher than those in the control group(P<0.05).All dimensions of SF-36 in the observation group were significantly better than those in the control group(P<0.05).Conclusion:Comprehensive nursing based on 5 E model for peritoneal dialysis patients has achieved remarkable results in controlling blood pressure,reducing cardiovascular complications and hospitalization times,it can also play a positive role in enhancing patients’ self-care ability,improve quality of life,and strengthen social functions.
