55例妊娠中期选择性减胎术后的妊娠结局与护理体会

Pregnancy outcomes and nursing management of 55 women with selective reduction in the second trimester of pregnancy

本文总结55例多胎妊娠孕妇妊娠中期选择性减胎术后的妊娠结局及护理措施。55例妊娠中期(妊娠13~22周)选择性减胎术的孕妇在腹部B超引导下经腹部行羊膜腔穿刺胎儿心内注射高浓度氯化钾选择性减胎,均给予基础护理、监测生命体征、观察腹痛及出血情况、心理护理、用药及饮食指导等,观察妊娠结局。55例孕妇中47例获得成活婴儿,其中获得足月儿孕妇40例,获得早产儿7例。研究中未见母体凝血功能障碍、保留胎儿损伤、生长受限等严重并发症出现。妊娠中期行选择性减胎术是一种安全可行的方法。通过对孕中期选择性减胎孕妇进行有效的护理干预,可明显提高手术成功率。

This paper summarized pregnancy outcomes and nursing management of 55 women with selective reduction in the second trimester of pregnancy. Totally 55 women with selective reduction in the second trimester of pregnancy(13-22 weeks)were selected as study objects. The ultrasound assisted selective reduction by intracardiac administration of Potassium Chloride was performed. All patients received comprehensive nursing interventions including monitoring vital signs, observing abdominal pain and bleeding, psychological and dietary guidance, etc. The occurrence of abortion rate, premature birth rate and live birth rate were analyzed. Of the 55 cases, 47(85.45%)had survived infants, including 40(72.73%)full-term infants and 7(27.27%)premature infants. There was no case of maternal coagulation disorders, fetal injury and fetal growth restriction reported. The selective reduction in the second trimester of pregnancy might be applied in clinic. Effective nursing intervention should be carried out to improve the success of selective reduction and safety of women during the pregnancy.
