实时剪切波弹性成像测量正常人不同臂展角度上斜方肌的弹性 被引量:4

Elasticity of the Upper Trapezius Muscle at Different Shoulder Abduction Angles Measured Using Real-Time Shear Wave Elastography

摘要 目的应用实时剪切波弹性成像测量正常人不同臂展角度上斜方肌的杨氏模量值,探讨实时剪切波弹性成像技术评价上斜方肌弹性的可行性及稳定性。方法纳入健康成年人240例,其中男138例,女102例,年龄20~60岁,平均年龄(35.5±12.2)岁。应用实时剪切波弹性成像测量参与者双侧上斜方肌不同臂展角度(0°、30°、60°、90°)下杨氏模量值,每名参与者均由2位超声医生测量,每位医生测量2次。结果双侧上斜方肌的杨氏模量值随臂展角度增加而增加,在0°、30°、60°、90°臂展角度下左侧上斜方肌的杨氏模量值分别为(16.88±5.32)、(26.34±3.15)、(30.11±5.32)、(42.39±3.54) kPa,右侧上斜方肌的杨氏模量值分别为(17.11±4.01)、(26.54±4.02)、(31.24±4.29)、(41.34±4.01) kPa。在0°、30°、60°、90°臂展角度下,左侧和右侧上斜方肌的杨氏模量比较,均无统计学差异(P> 0.05)。随着臂展角度的增加,上斜方肌的杨氏模量不断增加,各角度之间的差异均有统计学意义(P <0.05)。操作者间上斜方肌的杨氏模量测量值无统计学差异(P> 0.05)。结论实时剪切波弹性成像可以用于上斜方肌弹性的评价,测量所得杨氏模量值较稳定,重复性好。上斜方肌的杨氏模量值随臂展角度增加而增加。 Objective To investigate the feasibility and stability of real-time shear wave elastography for evaluating the elasticity of the upper trapezius muscle by measuring the Young’s modulus of the upper trapezius muscle at different shoulder abduction angles. Methods We enrolled 240 healthy adults,including 138 men and 102 women,aged 20-60 years(mean,35.5 ± 12.2 years). Real-time shear wave elastography was used to measure the Young’s modulus values of the bilateral upper trapezius muscles at different shoulder abduction angles(0°,30°,60°,and 90°). The measurement was performed twice by each of the two ultrasonography physicians. Results The Young’s modulus of both the superior trapezius increased with the increase in shoulder abduction angle. The Young’s modulus value of the left superior trapezius was 6.88 ± 5.32,26.34 ± 3.15,30.11 ± 5.32,and 42.39 ± 3.54 kPa and that of the right superior trapezius was 17.11 ± 4.01,26.54 ± 4.02,31.24 ± 4.29,and 41.34 ± 4.01 kPa at 0°,30°,60°,and 90° shoulder abduction angles,respectively. No significant differences in the Young’s modulus were found between the left and right superior trapezius muscles at 0°,30°,60°,and 90° shoulder abduction angles. With the increase in the shoulder abduction angles,the Young’s modulus of the superior trapezius muscle increased continuously,and the differences for all the angles were statistically significant. No significant difference in the Young’s modulus of the superior trapezius was observed between the two operators. Conclusion Real-time shear wave elastography can be used to evaluate the elasticity of the trapezius muscle. The Young’s modulus of the trapezius increase with the increase in shoulder abduction angle.
