降钙素原C-反应蛋白及白细胞计数联合检测在成人呼吸道疾病中的临床应用 被引量:3

摘要 目的探讨降钙素原、C-反应蛋白及白细胞计数联合检测在成人呼吸道疾病中的临床应用价值。方法选取我院2016年3月—2018年11月收治的成人呼吸道疾病患者150例,分为甲组慢性支气管炎急性发作(AECB)50例,属细菌感染;乙组社区获得性肺炎(CAP)50例;丙组为慢性阻塞性肺疾病急性加重(AECOPD)50例,属病毒感染。对3组患者的CRP、PCT以及WBC等进行检测。结果入院时,乙组患者的PCT水平最高,甲丙2组患者次之(P<0.05);乙组患者的CR P水平最高,其次为丙组,最低甲组(P<0.05);乙组患者WBC水平最高,而甲丙2组水平无明显差异(P>0.05)。出院后3组CRP、PCT以及WBC均降低,且乙组降低最为明显(P<0.05)。结论对于呼吸道感染患者诊断中,CRP、PCT以及WBC对于病情评估存在一定的指导作用,特别是CAP对于细菌性感染的诊断更为敏感。 Objective To investigate the clinical application of combined detection of procalcitonin,C-reactive protein and white blood cell count in adult respiratory diseases.Methods One hundred and fifty patients with adult respiratory diseases admitted to our hospital from March 2016 to November 1818 were enrolled as a study group. Group A was an acute exacerbation of chronic bronchitis (AECB),which was a bacterial infection.Group B was 50 cases of community acquired pneumonia (CAP).Group C was 50 cases of acute exacerbation of chronic obstructive pulmonary disease (AE-COPD),which was a viral infection.CRP,PCT,and WBC were tested before and after admission in three groups of patients.Results Patients in group B had the highest PCT levels,followed by patients in both groups. Group B patients had the highest CRP levels,followed by group C and group A. The WBC level was also the highest in group B,but there was no significant difference between the two groups (P<0.05).Among the three groups of CRP,PCT and WBC after discharge,group B decreased most significantly (P<0.05).Conclusion In the diagnosis of patients with respiratory infection,CRP,PCT and WBC have a certain guiding role in the assessment of disease,especially CAP is more sensitive to the diagnosis of bacterial infection.
