中医特色护理干预在全髋关节置换术患者中的护理效果及对肢体功能的影响研究

Study on the Nursing Effect of the Nursing Intervention with TCM Characteristics and Its Effect on the Limb Function in Patients with Total Hip Arthroplasty

目的研究中医特色护理干预在全髋关节置换术患者中的护理效果及对肢体功能的影响。方法选取2016年1月到2017年1月于西南医科大学附属医院实施髋关节置换术的100例患者,随机分为试验组和对照组,每组各50例,对照组实施普通护理,试验组实施具有中医特色的护理,手术后第7、14天对两组患者的髋关节功能进行Harris评分,与术前评分进行比较。同时,比较两组患者的住院天数和并发症发病率。结果治疗后试验组髋关节功能Harris评分明显高于对照组(P<0.05);试验组患者住院天数明显短于对照组(P<0.05);术后并发症包括深静脉血栓、肺部感染和关节脱位,试验组术后1个月并发症发生率明显少于对照组(P<0.05)。结论全髋关节置换术患者采用特色的中医护理干预,较常规护理有更好的恢复效果,住院时间和术后并发症的发生率均明显降低,提高了患者的治疗体验和满意程度,减轻了患者痛苦。

Objective To study the nursing effect of the nursing intervention with TCM characteristics and its effect on the limb function in patients with total hip arthroplasty. Methods A total of 100 patients undergoing the hip arthroplasty in The Affiliated Hospital of Southwest Medical University from January of 2016 to January of 2017 were selected and randomly divided into the experiment group and control group.The control group was treated with the conventional nursing while the experiment group was given the nursing with TCM characteristics.The hip joint function of the two groups was assessed by Harris hip scores before operation and at the 7th and 14th day after operation and the Harris hip scores after operation were compared with those before operation.Meanwhile, the hospitalization days and incidence of complications were compared between the two groups. Results After treatment, the Harris hip score in the experiment group was significantly higher than that in the control group ( P <0.05) and the hospitalization days were significantly less in the experiment than in the control group ( P <0.05).The postoperative complications involved deep venous thrombosis, pulmonary infection and joint dislocation, and the incidence of complications 1 month after operation was significantly less in the experiment group than in the control group ( P <0.05). Conclusion The nursing intervention with TCM characteristics has better recovery effect than the conventional nursing in patients with total hip arthroplasty, and it can significantly reduce the hospitalization days and incidence of complications, improve patients' treatment experience and satisfaction and alleviate their pain.
