病灶扩大刮除联合软骨下植骨、骨水泥填充治疗膝关节周围骨巨细胞瘤的临床效果

Clinical effect of extended curettage combined with subchondral bone grafting and cement filling in the treatment of giant cell tumor of bone around knee joint

目的探讨病灶扩大刮除联合软骨下植骨、骨水泥填充治疗膝关节周围骨巨细胞瘤(GCT)的临床效果。方法选取2009年2月~2019年2月我院收治的60例膝关节周围GCT患者作为研究对象,采用随机数字表法分为对照组(病灶扩大刮除术)和观察组(病灶扩大刮除术联合软骨下植骨、骨水泥填充治疗),每组各30例。比较治疗后两组的Enneking肢体功能评分、应用可行性及术后并发症、复发率等情况。结果治疗后,观察组的Enneking肢体功能总体评分高于对照组,差异有统计学意义(P<0.05);观察组的并发症总发生率为6.67%,低于对照组的36.67%,差异有统计学意义(P<0.05);观察组的复发率为3.33%,低于对照组的16.67%,差异有统计学意义(P<0.05)。结论对膝关节周围GCT患者,采取病灶扩大刮除术联合软骨下植骨、骨水泥填充治疗,可有效改善患者的生活质量,提高患者肢体功能恢复的速度,并发症及复发率较低,有较高临床应用价值。

Objective To investigate the clinical effect of extended curettage combined with subchondral bone grafting and cement filling in the treatment of giant cell tumors (GCT) of bone around the knee joint.Methods From February 2009 to February 2019,60 patients with GCT of bone around the knee joints in our hospital were selected and divided into the control group (focal enlargement curettage) and the observation group (focal enlargement curettage combined with subchondral bone grafting and bone cement filling),30 cases in each group.Enneking limb function score,feasibility of application,postoperative complications and recurrence rate were compared between two groups.Results After treatment,the Enneking limb function score of the observation group was higher than that of the control group,with statistical significance (P<0.05);the total incidence of complications in the observation group was 6.67%,lower than 36.67% of the control group,with statistical significance (P<0.05);the recurrence rate of the observation group was 3.33%,lower than 16.67% of the control group,with statistical significance (P<0.05).Conclusion For patients with GCT of bone around the knee joint,enlarged curettage combined with subchondral bone grafting and bone cement filling can effectively improve the quality of life,speed up the recovery of limb function,lower complications and recurrence rate,and have higher clinical application value.
