超声弹性成像联合新版BI-RADS分类诊断乳腺癌的价值 被引量:1

摘要 目的探讨超声弹性成像(UE)对乳腺包块新版乳腺影像报告和数据系统(BI-RADS)分类的诊断价值。方法选取2016年10月~2018年10月我院接受乳腺超声检查的105例患者共有包块113个,进行乳腺包块BI-RADS分类及UE,均进行以病理结果并以此作为金标准,比较分析新版BI-RADSSR分类与UE联合新版BI-RADS分类对乳腺包块良、恶性诊断的灵敏度、特异性及准确度。结果病理诊断作为诊断良、恶性包块的标准,UE联合新版BI-RADS分类的诊断准确度为81.42%(92/113),特异性为76.92%(60/78),均高于新版BI-RADS分类诊断的68.14%(77/113)、61.54%(48/78),差异有统计学意义(P<0.05);UE联合新版BI-RADS分类的灵敏度为91.43%(32/35),与新版BI-RADS分类的82.86%(29/35)比较,差异无统计学意义(P>0.05)。结论UE联合新版BI-RADS分类能显著提高乳腺包块良、恶性的准确度,而避免患者进行不必要的穿刺活检。 Objective To investigate the diagnostic value of ultrasound elastography (UE) in breast mass classification by the new breast mass image reporting and data system (BI-RADS).Methods A total of 113 breast masses were selected from 105 patients who underwent breast ultrasound examination in our hospital from October 2016 to October 2018 BI-RADS and UE were classified.The pathological results were used as the gold standard to compare and analyze the new BI-RADSSR classification and UE combined BI-RADS classification for breast cancer.Sensitivity,specificity and accuracy in the diagnosis of benign and malignant adenomas were compared.Results The diagnostic accuracy of UE combined with BI-RADS was 81.42%(92/113) and the specificity was 76.92%(60/78),which were higher than 68.14%(77/113) and 61.54%(48/78) of the new BI-RADS classification (P<0.05).The sensitivity of UE combined with new BI-RADS classification was 91.43%(32/35),compared with the new BI-RADS classification 82.86%(29/35) with no significant difference (P>0.05).Conclusion UE combined with new BI-RADS classification can significantly improve the accuracy of benign and malignant breast masses,thus avoiding needless biopsy.
