Analysis of the Economic Burden of School Aggregated Varicella in 2017

目的分析2017年上海市徐汇区管辖的托幼和学校集体机构的水痘患者的疾病经济负担及其影响因素,为卫生行政管理部门提供卫生监督管理工作提供方向。方法收集2017年7月1日—12月31日时间内《中国疾病预防控制信息系统》报告的隶属于上海市徐汇区管辖的托幼和学校集体机构的所有水痘病例共171例作为研究对象。通过《集体机构水痘病例临床表现、流行病史及费用支出个案调查表》问卷收集患者的基本信息、就诊情况、直接经济负担和间接经济负担。结果结果显示:本地患者的直接医疗费用显著高于外来患者,存在显著性差异;年龄<15岁患者的间接费用和总费用显著高于年龄≥15的患者;就诊次数≥2患者的直接医疗费用、直接非医疗费用、间接费用,总费用显著高于就诊次数<2的患者;就诊医院级别为三甲医院及以上患者的直接医疗费用和直接非医疗费用显著高于就诊医院级别低于三甲医院的患者。结论上海市徐汇区因患水痘造成患者经济负担加重。病例属地、年龄、就诊次数和就诊医院等级是影响水痘经济负担的主要因素。

Objective To analyze the economic burden and its influencing factors of varicella patients in kindergartens and school collective institutions under the jurisdiction of Xuhui District in Shanghai in 2017, and to provide the directions for health supervision and management for health administration departments. Methods A total of 171 all varicella cases in kindergartens and school collective institutions under the jurisdiction of Xuhui District in Shanghai were reported by the “China Disease Prevention and Control Information System” from July 1,2017 to December 31 of 2017 were selected as subjects.The basic information, seek medical condition,direct and indirect financial burdens of patients were collected through the questionnaire on clinical manifestations,epidemiological history and expenditure of varicella cases in collective institutions. Results The results showed that the direct medical expenses of local patients were significantly higher than those of foreign patients,and there were significant differences;the indirect and total costs of patients aged <15 years were significantly higher than those of patients aged <15 years;the direct medical expenses,direct non-medical expenses,and indirect expenses of patients whose times of seek medical were more than 2 were significantly higher than those of patients whose times of seek medical were less than 2.The direct medical expenses and direct non-medical expenses of patients in grade A hospitals and above were significantly higher than those of patients in lower grade of hospital than Grade A.Conclusion The economic burden of patients suffering from varicella in Xuhui District of Shanghai is aggravated.The nature of the case,age,number of visits, and hospital grade are the main factors affecting the economic burden of varicella.
