摘要
目的:本研究旨在探讨常规医疗技术--肌肉松解术与普拉提斯运动锻炼及两者相结合的治疗方案分别对中青年患者腰肌劳损的长期治疗效果。方法:选取南京市6家医院因腰肌劳损就诊的20-50周岁患者100例,随机分为肌肉松解术治疗组(A组)、普拉提斯运动锻炼治疗组(B组)、肌肉松解术与普拉提斯运动锻炼结合治疗组(C组)和常规针灸推拿治疗组(D组),每组25例。每次干预60min,每周干预3次,分别在干预前、干预后3、6和12个月评估患者的腰肌疼痛强度和腰椎活动度。结果:(1)经过3个月治疗后,肌肉松解术与普拉提斯运动锻炼结合治疗组患者各项指标均显著优于治疗前(P<0.05),6个月后,该组各项指标依然显著好于其他各组;肌肉松解组和常规针灸推拿组各项指标均比较接近,好于普拉提斯运动锻炼组;12个月后,肌肉松解术与普拉提斯运动锻炼结合治疗组各项指标依然显著好于其他各组;(2)是普拉提斯运动锻炼组,要优于肌肉松解组和常规针灸推拿组,但普拉提斯运动锻炼组和其他各组的差异不显著;肌肉松解组和常规针灸推拿组各项指标接近。结论:肌肉松解术结合普拉提斯运动锻炼对于中青年患者腰肌劳损的治疗效果最佳。
Objective: the purpose of this study was to investigate the long-term therapeutic effects of muscle relaxation and pilates exercise combined with conventional medical techniques on lumbar muscle strain in young and middle-aged patients. Methods: A total of 100 patients aged 20-50 years old who were treated for lumbar muscle strain in 6 hospitals in nanjing were randomly divided into muscle loosening treatment group(group A), pilates exercise treatment group(group B), muscle loosening combined with pilates exercise treatment group(group C) and conventional acupuncture and massage treatment group(group D), each of which had 25 cases. Psoas pain intensity and lumbar mobility were assessed at 1000 days before and 3, 6, and 12 months after the intervention. Results :(1) after 3 months of treatment, all indexes of patients in the treatment group combined with muscle relaxation and pilates exercise were significantly better than those before treatment(P<0.05), and all indexes in the treatment group were still significantly better than those in other groups after 6 months. The indexes of muscle relaxation group and conventional acupuncture and massage group were close, better than pilates exercise and plating group. After 12 months, the indexes of muscle relaxation combined with pilates exercise group were still significantly better than other groups.(2) the pilates exercise group was better than the muscle relaxation group and the conventional acupuncture and massage group, but the pilates exercise group was not significantly different from other groups;The indexes of muscle relaxation group and conventional acupuncture and massage group were close to each other. Conclusion: muscle relaxation combined with pilates exercise has the best therapeutic effect on lumbar muscle strain in young and middle-aged patients.
