网络管理平台在预防草酸钙结石患者复发中的应用

目的探讨网络管理平台在预防草酸钙结石患者复发中的作用。方法选取我院2016年1月~2017年12月期间收住的草酸钙结石患者160例,随机分为实验组和对照组,每组80例,观察组在对照组的基础上运用网络管理平台帮助患者进行饮食管理。6个月及12个月时复查B超了解结石复发情况,并监测尿液一般性指标(尿量、pH值)、抑制成石因子(葡胺聚糖、尿凝血酶原片段)、促进成石因子(钙离子、尿酸、草酸)。结果 6个月和12个月时实验组结石复发率分别为5.00%和8.97%,较对照组的15.00%和21.05%明显降低(P<0.05);各时间点实验组24 h尿量较对照组增加(P<0.05);24 h尿钙、尿酸、尿草酸浓度均较对照组降低(P<0.05);尿凝血酶原片段-1浓度较对照组增加(P<0.05);尿pH值、尿葡胺聚糖含量与对照组相比无明显差异(P>0.05)。结论通过网络管理平台的使用,以个体化饮食健康处方的形式帮助草酸钙结石患者进行饮食管理,强化健康教育知识,建立良好的生活方式,能有效预防草酸钙结石的复发,具有临床推广价值。

Objective To explore the role of online management platform in preventing recurrence of calcium oxalate stones. Methods A total of 160 patients with calcium oxalate stones, admitted to our hospital from January 2016 to December 2017, were recruited and randomly divided into experimental group and control group, with 80 cases in each group. The experimental group was given online platform management to help manage their diet on top of routine management. Patients were followed up 6 and 12 months later with B-ultrasound imaging to identify recurrence of stones,alongside with other measures on general urine indicators(urine volume and pH), inhibitive factors(glycosaminoglycan and urinary prothrombin fragment) and enabling factors(calcium, uric acid, and oxalic acid) for stone formation. Results Recurrence rates of calculi in the experimental group were 5.00% and 8.97% at 6 and 12 months, respectively, significantly lower than those(15.00%, 21.05%) in the control group(P<0.05). 24-hour urine volume in the experimental group was higher than that in the control group at each study point(P<0.05);the 24-hour urinary calcium, uric acid, and urinary acid concentrations were lower in the experimental group than that in the control group(P<0.05);higher urinary prothrombin fragment-1 concentration was observed in the experimental group than that in the control group(P<0.05);there was no significant difference between the two groups in urine pH and urinary glycosaminoglycan(P>0.05). Conclusion Online management platform can effectively prevent the recurrence of calcium oxalate stone by delivering individualized dietary health prescriptions to help patients with calcium oxalate stones manage diet, enhance health awareness and live a better lifestyle, making it worthy of wider clinical application.
