视频脑电图联合脑干听觉诱发电位在新生儿脑膜炎预后评估的价值临床研究

The value of video electroencephalogram combined with brainstem auditory evoked potential in the evaluation of neonatal meningitis prognosis

目的:探究视频脑电图(VEEG)联合脑干听觉诱发电位(BAEP)在新生儿化脓性脑膜炎预后评估中的价值临床研究。方法:选取本院新生儿科2018年4月至2019年1月收治的42例新生儿化脓性脑膜炎患儿,入院后对患儿进行视频脑电图(VEEG)、脑干听觉诱发电位(BAEP)、头颅MRI检查及新生儿神经行为测定(NBNA)评分,对比患儿出院1个月、出院6个月的Gesell评估。结果:42例患儿中VEEG及BEAP均异常者为30例,除Gesell评估未完成外,该30例患儿有28例存有NBNA评分或头颅MR检查中的一项或两项异常,其中有22例完成1个月Gesell评估,20例存在异常,11例完成6个月Gesell评估,8例均存在异常。VEEG、BAEP及VEEG+BAEP与新生儿化脓性脑膜炎之间存在显著相关性。结论:视频脑电图(VEEG)联合脑干听觉诱发电位(BAEP)在新生儿化脓性脑膜炎预后的价值高,疗效确切,值得临床上进一步研究。

Objective:To explore the value of video electroencephalogram(VEEG)combined with brainstem auditory evoked potential(BAEP)in the evaluation of neonatal meningitis prognosis.Methods:Forty-two cases of neonatal purulent meningitis were selected from April 2018 to January 2019.After the children'admission to the hospital,their VEEG,BAEP,skull MRI and neonatal neurobehavioral measurement(NBNA)scores were determined,with Gesell evaluation compared 1 month and 6 months after discharge.Results:30 out of the 42 children had VEEG and BEAP abnormalities;28 out of the 30 children had one or two abnormalities in the NBNA score or head MR examination except that Gesell's assessment was not finished;Gesell's assessment for 22 of them was finished in one month,20 of whom were abnormal;the Gesell assessment for 11 of them was finished in six months,8 of whom were abnormal.There was significant correlation between VEEG,BAEP and VEEG+BAEP and neonatal purulent meningitis.Conclusion:The value of video electroencephalogram(VEEG)combined with brainstem auditory evoked potential(BAEP)in the prognosis of neonatal meningitis is high and the curative effect is accurate,which is worth further clinical study.
