摘要 目的探讨胸部综合物理治疗对机械通气患者脱机趋势的影响。方法选取ICU机械通气患者60例,采用随机数字表法分为观察组(32例)和对照组(28例)。对照组采取常规治疗护理,观察组采用胸部综合物理治疗。收集并分析两组浅快呼吸指数(RSBI)、氧合指数(OI)、肺不张、机械通气时间的变化。结果干预第3天,两组RSBI、OI比较,差异有统计学意义(均P<0.01);观察组肺不张发生例数少于对照组,机械通气时间短于对照组,差异无统计学意义(均P>0.05)。结论基于病情评估的胸部物理治疗可有效改善机械通气患者肺氧合功能,减少肺不张发生概率,对加快脱机可能有积极影响,但仍需通过进一步研究证实。 Objective To investigate the effect of comprehensive chest physical therapy on weaning trend of patients on mechanical ventilation therapy.Methods Totally,sixty patients on mechanical ventilation therapy in the ICU were randomly divided into an intervention group(n=32)and a control group(n=28).The control group received routine treatment and the intervention group received comprehensive chest physical therapy.Changes in rapid shallow breathing index(RSBI),and oxygenation index(OI),incidence rate of atelectasis and length of mechanical ventilation therapy were collected and analyzed for the two groups.Results There were significant differences in RSBI and OI,between the two groups on the 3 rd day of intervention(P<0.01 for both);the incidence rate of atelectasis in the intervention group was lower than in the control group,and the duration of mechanical ventilation in the former were shorter than in the latter,though no significant differences were found(P>0.05 for both).Conclusion Comprehensive chest physical therapy based on disease assessment can effectively improve pulmonary oxygenation function and reduce pulmonary complications,which may have a positive effect on accelerating weaning and needs to be confirmed through further research.
