成人双颌前突正畸中采用微螺钉种植体支抗治疗的临床效果

Clinical effect of micro-screw implant anchorage in adult bimaxillary protrusion orthodontics

目的探讨成人双颌前突正畸中采用微螺钉种植体支抗治疗的临床效果。方法选取2010年8月至2017年8月我院收治的成人双颌前突患者80例,随机将其分为研究组(40例,微螺钉种植体支抗正畸治疗)与对照组(40例,传统方式支抗正畸治疗)。比较两组的矫治各个阶段时间(排齐整平时间、精细调整与被动保持时间、关闭拔牙间隙时间及主动矫治时间)、矫治前、后的蝶胺中心、鼻根点及上牙槽座点交角(SNA)、上中切牙角(U1-SN)、下中切牙角(L1-MP)、上下中切牙长轴交角(U1-LI)及并发症(矫治器松动、矫治器脱落、口腔肿胀)发生率。结果两组的排齐整平时间、精细调整与被动保持时间比较,差异无统计学意义(P>0.05);研究组的关闭拔牙间隙时间、主动矫治时间均显著短于对照组,差异具有统计学意义(P<0.05)。矫治后,两组的U1-SN、L1-MP均显著降低,U1-LI均显著升高,且研究组优于对照组,差异具有统计学意义(P<0.05)。矫治后,两组SNA比较,差异无统计学意义(P>0.05)。研究组并发症总发生率为5.0%,略低于对照组的12.5%(P>0.05)。结论成人双颌前突正畸中采用微螺钉种植体支抗治疗的临床效果显著,能缩短矫治时间,提高矫治效果,值得在临床推广应用。

Objective To investigate the clinical effect of micro-screw implant anchorage in adult bimaxillary protrusion orthodontics.Methods A total of 80 patients with adult bimaxillary protrusion in our hospital from August 2010 to August 2017 were randomly divided into study group(40 cases,orthodontic treatment of micro-screw implant anchorage)and control group(40 cases,traditional anchorage orthodontic treatment).The time of each stage of orthodontic treatment(arrangement time,fine adjustment and passive retention time,closure of extraction space time and active correction time),the angle of butterfly amine center,nasal root and upper alveolar base(SNA),upper central incisor angle(U1-SN),lower central incisor angle(L1-MP),longitudinal angle of upper and lower central incisors(U1-LI)before and after orthodontic treatment and incidence of complications(loosening and falling off of appliance,swollen mouth).Results There were no significant differences in the arrangement time,fine adjustment and passive retention time between the two groups(P>0.05);the closure of extraction space time and active correction time in the study group were significantly shorter than those in the control group,and the differences were statistical significant(P<0.05).After treatment,the U1-SN and L1-MP in both groups decreased significantly,and the U1-LI increased significantly,and those of the study group were better than the control group,and the differences were statistically significant(P<0.05).After treatment,there was no significant difference in SNA between the two groups(P>0.05).The total incidence of complications in the study group was 5.0%,which was slightly lower than 12.5%in the control group(P>0.05).Conclusion Micro-screw implant anchorage applied to adult bimaxillary protrusion orthodontics has a significant effect,it can shorten the treatment time,and improve the treatment effect,which is worthy of clinical promotion and application.
