美容缝合术与常规缝合术联合重组人表皮生长因子治疗颌面部外伤的效果

Effects of cosmetic suture and conventional suture combined with recombinant human epidermal growth factor in the treatment of maxillofacial trauma

目的比较不同清创缝合术联合重组人表皮生长因子治疗颌面部外伤的效果。方法选取本院2017年9月至2018年9月收治的120例颌面部外伤患者为研究对象,随机将其分成观察组和对照组,各60例。对照组采用常规缝合术联合重组人表皮生长因子治疗,观察组采用美容缝合术联合重组人表皮生长因子治疗。比较两组患者术前及术后3、6d血清表皮生长因子、炎性因子水平、平均愈合时间、术后瘢痕长度、术后伤口感染率和手术满意度。结果术前及术后3、6d,两组的血清表皮生长因子、TNF-α、IL-6和IL-1水平比较,差异不具有统计学意义(P>0.05);术后3d,两组的血清表皮生长因子、TNF-α、IL-6和IL-1水平均较术前改善(P<0.05)。观察组的平均愈合时间、术后瘢痕长度均显著短于对照组,差异具有统计学意义(P<0.05)。观察组的术后伤口感染率显著低于对照组,手术满意度显著高于对照组,差异具有统计学意义(P<0.05)。结论美容缝合术联合重组人表皮生长因子治疗颌面部外伤,可在保证功能完整性的同时达到良好的美容效果,值得推广应用。

Objective To compare the effects of different debridement and suture combined with recombinant human epidermal growth factor in the treatment of maxillofacial trauma.Methods A total of 120 patients with maxillofacial trauma admitted to our hospital from September 2017 to September 2018 were selected as the research objects and randomly divided into observation group and control group,with 60 cases in each group.The control group was treated with conventional suture combined with recombinant human epidermal growth factor,while the observation group was treated with cosmetic suture combined with recombinant human epidermal growth factor.The levels of serum epidermal growth factor,inflammatory factors before operation and at 3 and 6 days after operation,average healing time,scar length,wound infection rate and surgical satisfaction were compared between the two groups.Results There were no significant differences in the levels of serum epidermal growth factor,TNF-α,IL-6 and IL-1 between the two groups before operation and at 3 and 6 days after operation(P>0.05);at 3 days after operation,the levels of serum epidermal growth factor,TNF-α,IL-6 and IL-1 in both groups improved compared with those before operation(P<0.05).The average healing time and scar length in the observation group were significantly shorter than those in the control group,the differences were statistically significant(P<0.05).The wound infection rate of the observation group was significantly lower than that of the control group,and the satisfaction rate of operation was significantly higher than that of the control group,the differences were statistically significant(P<0.05).Conclusion Cosmetic suture combined with recombinant human epidermal growth factor in the treatment of maxillofacial trauma can ensure functional integrity and achieve good cosmetic effect,which is worthy of promotion and application.
