64排螺旋CT增强扫描在胃上皮内瘤变和早期胃癌诊断中的应用价值

Application value of 64-slice spiral CT enhanced scanning in the diagnosis of gastric intraepithelial neoplasia and early gastric cancer

目的探讨64排螺旋CT增强扫描在胃上皮内瘤变和早期胃癌诊断中的应用价值。方法回顾性分析2016年7月至2018年6月在我院行64排螺旋CT增强扫描的胃上皮内瘤变和早期胃癌患者(各50例)的影像学资料,并选择50例健康志愿者作为对照组。比较三组受检者的胃壁黏膜厚度和增强各期ΔCT值。结果三组受检者的胃壁黏膜厚度无显著差异(P>0.05)。胃上皮内瘤变组的动脉值显著高于早期胃癌组和对照组,差异具有统计学意义(P<0.05);胃上皮内瘤变组和早期胃癌组的门脉值、平衡值均显著高于对照组,差异具有统计学意义(P<0.05)。胃上皮内瘤变组的平衡值显著低于门脉值,差异具有统计学意义(P<0.05);早期胃癌组的动脉值及平衡值均显著低于门脉值(P<0.05)。结论64排螺旋CT增强扫描检查胃上皮内瘤变和早期胃癌的ΔCT值有显著差异,同时其具有扫描速度快、范围大、空间分辨率高,图像清晰的优势,可为早期胃癌和胃上皮内瘤变的早期诊断和鉴别诊断提供依据。

Objective To investigate the application value of 64-slice spiral CT enhanced scanning in the diagnosis of gastric intraepithelial neoplasia and early gastric cancer.Methods The imaging data of gastric intraepithelial neoplasia(n=50)and early gastric cancer patients(n=50)who underwent 64-slice spiral CT enhanced scanning in our hospital from July 2016 to June 2018 were retrospectively analyzed,and 50 healthy volunteers were selected as control group.The thickness of gastric mucosa and the enhanced CT values at different stages were compared among the three groups.Results There was no significant difference in the thickness of gastric mucosa between the three groups(P>0.05).The arterial value of the gastric intraepithelial neoplasia group was significantly higher than that of the early gastric cancer group and control group,the differences were statistical significance(P<0.05);the portal vein values and balance valuea of the gastric intraepithelial neoplasia group and early gastric cancer group were significantly higher than those of the control group,and the differences were statistical significance(P<0.05).In the gastric intraepithelial neoplasia group,the balance value was significantly lower than the portal vein value,the difference was statistically significant(P<0.05);in the early gastric cancer group,the arterial value and balance value were significantly lower than the portal vein value(P<0.05).Conclusion The 64-slice spiral CT enhanced scanning in detecting gastric intraepithelial neoplasia and early gastric cancer has significantly differentΔCT value.It has the advantages such as fast scanning speed,wide range,high spatial resolution and clear image,which can provide basis for early diagnosis and differential diagnosis of early gastric cancer and gastric intraepithelial neoplasia.
