多囊卵巢综合征患者与输卵管性不孕患者子宫内膜Survivin表达的对比分析

Comparative analysis of endometrial Survivin expression in patients with polycystic ovary syndrome and tubal infertility

目的探讨与对比多囊卵巢综合征(PCOS)患者与输卵管性不孕患者子宫内膜Survivin的表达情况。方法选取2013年2月至2017年1月十堰市人民医院(湖北医药学院附属人民医院)诊治的120例PCOS患者作为研究对象。将这120例PCOS患者设为PCOS组,同期选择在十堰市人民医院(湖北医药学院附属人民医院)诊治的输卵管性不孕患者120例作为不孕组,并选择在十堰市人民医院(湖北医药学院附属人民医院)体检的120例健康人作为对照组。三组都选择子宫内膜诊治进行Survivin表达的免疫组化分析,并进行相关临床资料的调查。结果 PCOS组与不孕组的Survivin表达阳性率分别为35.8%和34.2%,都明显高于对照组的3.3%,差异具有统计学意义(P<0.05),PCOS组与不孕组对比差异无统计学意义(P>0.05)。直线相关分析显示PCOS组患者的Survivin表达阳性率与病程、体重指数、空腹血糖、瘦素等指标明显相关,差异具有统计学意义(P<0.05);不孕组患者的Survivin表达阳性率与病程、瘦素等指标明显相关,差异具有统计学意义(P<0.05);logistic回归分析结果显示病程、瘦素都为影响Survivin表达阳性独立危险因素,差异具有统计学意义(P<0.05)。结论 PCOS患者与输卵管性不孕患者中子宫内膜Survivin都呈现高表达状况,与患者的病程与瘦素分泌有明显相关性,可以作为评价患者病情的标志之一。

Objective To investigate and compare the expression of Survivin in endometrium of patients with polycystic ovary syndrome(PCOS) and tubal infertility. Methods 120 PCOS patients in Shiyan Renmin Hospital from February 2013 to January 2017 were selected as PCOS group and 120 patients with tubal infertility and other 120 healthy people for physical examination in the hospital during the same period were selected as the infertility group and the control group respectively. The endometrial Survivin expression analysis was performed and clinical data was investigated. Results The positive rates of Survivin expression in PCOS group and infertility group was 35.8% and 34.2% respectively, which was obviously higher than the 3.3% in control group(P<0.05), and there was no significant difference between PCOS group and infertility group(P>0.05). Linear correlation analysis showed that the positive rate of Survivin expression in PCOS patients was closely related to the course of disease, body mass index, fasting blood glucose and leptin(P<0.05);the positive rate of Survivin expression ininfertility patients was significantly related with the course of disease and leptin(P<0.05). Logistic regression analysis results showed that the course of disease and leptin were all independent risk factors affecting the expressionof Survivin, with statistically significant differences(P<0.05). Conclusions Endometrial Survivin are highly expressed in patients with PCOS and tubal infertility. It is closely related to the course of disease and leptin secretion, which can be used as a marker to evaluate the condition of patients.
