三种不同喉镜在困难气道人体模型上气管插管的对照研究

Comparative Study of Endotracheal Intubation in a Human Model of Difficult Airway with Three Different Laryngoscopes

目的探讨“Nishikawa镜片”、“McCoy镜片”、“Macintosh镜片”这三种不同喉镜用于困难气道人体模型上气管插管的情况。方法30名无临床气管插管操作经验的实习医生在困难气道人体模型上,分别采用这三种不同喉镜进行气管插管。分析气管插管的成功率、气管插管所需要的时间、声门暴露程度这三种指标。结果“Nishikawa镜片”喉镜、“McCoy镜片”喉镜、“Macintosh镜片”喉镜气管插管的成功率分别为100%、93.33%、83.33%。“Nishikawa镜片”与“Mc‐Coy镜片”喉镜的气管插管的成功率之间差异无统计学意义(χ^2=2.842;P>0.05)而“Nishikawa镜片”喉镜的气管插管的成功率显著大于“Macintosh镜片”喉镜(χ^2=5.364;P<0.05);结论“Nishikawa镜片”喉镜较为容易被初学者掌握;“Nishikawa镜片”喉镜可作为困难气道气管插管的合适选择之一。

Objective To investigate the endotracheal intubation in a difficult airway using three different laryngoscopes:“Nishikawa lens”,“McCoy lens”and“Macintosh lens”.Methods Endotracheal intubation was performed on a difficult airway by 30 interns without clinical experience of endotracheal intubation.The success rate of endotracheal intubation,time needed for endotracheal intubation and glottis exposure were analyzed.Results The success rates of endotracheal intubation of “Nishikawa lens” laryngoscope,“McCoy lens”laryngoscope and “Macintosh lens” laryngoscope were 100%,93.33% and 83.33%,respectively.There was no significant difference in the success rate of endotracheal intubation between “Nishikawa lens” and “McCoy lens” laryngoscope(χ^2=2.842;P>0.05).The success rate of endotracheal intubation of “Nishikawa lens”laryngoscope was significantly higher than that of “Macintosh lens” laryngoscope(χ^2=5.364;P<0.05).Conclusion “Nishikawa lens”laryngoscope was easily mastered by beginners.“Nishikawa lens” laryngoscope can be used as one of the appropriate choices for difficult airway endotracheal intubation.
