Clinical pharmacists participating in the drug treatment for Crohn’s disease

目的探讨临床药师参与克罗恩病的药物治疗实践方法。方法临床药师从治疗过程的药物选择、药学监护、营养治疗及用药教育等多方面入手,对患者用药进行分析,并为医师、护士和患者提供药学服务。结果临床药师依据患者特点,协助医师和护士制订了合理有效的用药方案,患者病情好转出院。结论临床药师在医药护组成的治疗团队中,充分发挥自身的专业特点,能保障患者用药安全、有效,提高临床治疗水平。

Objective To explore the practice method of clinical pharmacists participating in clinical drug treatment for Crohn's disease. Methods Clinical pharmacists analyzed the patients' medication and provided pharmaceutical care for physicians,nurses and patients in terms of the drug selection,pharmaceutical care,nutritional therapy and medication education in the treatment process. Results Clinical pharmacists helped physicians and nurses to formulate reasonable and effective treatment schemes according to the characteristics of patients, and the patients were discharged with a better health condition. Conclusion Clinical pharmacists in the medical care team give full play to their professional characteristics to assist physicians and nurses,which can ensure the safe and effective use of drugs and improve the level of clinical treatment.
