摘要
目的:研究超声心动图血流参数指标在预测早产儿动脉导管自然关闭中的应用效果。方法:回顾性分析出生24 h内进行超声心动图检查且存在动脉导管未闭(PDA)的168例早产儿,根据动脉导管关闭情况,将出生8 d内的33例PDA早产儿纳入PDA组,将135例自然关闭早产儿纳入关闭组。两组分别于出生后3 d及8 d对相同早产儿再次行超声心动图检查,对比两组24 h内行超声心动图测得的肺动脉收缩压、卵圆孔分流流速及收缩期肺动脉与降主动脉间的脉压差。结果:PDA组收缩期肺动脉与降主动脉间的脉压差明显低于关闭组,其差异有统计学意义(t=7.51,P<0.05);PDA组肺动脉收缩压明显高于关闭组,其差异有统计学意义(t=5.483,P<0.05);PDA组卵圆孔分流流速明显低于关闭组,其差异有统计学意义(t=6.036,P<0.05)。结论:超声心动图血流参数指标在预测早产儿动脉导管是否自然关闭中有较好的应用效果,具有临床应用价值。
Objective: To investigate the application effect of blood flow parameters of echocardiogram in predicting the natural closure of ductus arteriosus of premature infant. Methods: The information of 168 premature infants with patent ductus arteriosus(PDA) who underwent echocardiogram examination within postpartum 24 h were retrospectively analyzed. And according to closed situation of ductus arteriosus 33 premature infants with PDA within postpartum 8 d were divided into PDA group, and 135 premature infants with natural closure were divided into closure group. These premature infants of two groups were implemented echocardiogram examination at postpartum 3 th d and 8 th d, respectively. And the pulmonary arterial systolic pressure, the shunt flow rate of oval foramen and the pulse pressure difference between pulmonary artery and descending aorta during contraction stage that were examined by echocardiogram within postpartum 24 h between two groups were compared. Results: The pulse pressure difference between pulmonary artery and descending aorta during contraction stage of PDA group was significantly lower than that of control group(t=7.51, P<0.05). And the pulmonary arterial systolic pressure of PDA group was significantly higher than that of closure group(t=5.483, P<0.05). Besides, the shunt flow rate of oval foramen of PDA group was significantly lower than that of closure group(t=6.036, P<0.05). Conclusion: The indicators of blood flow parameters of echocardiogram have better application effect in predicting whether the ductus arteriosus of premature infant is naturally closed, and it has value in clinical application.
