普通血液透析与高通量血液透析对铁调素及相关因素的影响对比 被引量:8

Comparison of the effects of ordinary hemodialysis and high-throughput hemodialysis on Hepc and its relevant factors

摘要 目的:研究对比普通血液透析与高通量血液透析对铁调素(Hepc)、血红蛋白(Hb)、铁蛋白(SF)以及C反应蛋白(CRP)水平的影响。方法:前瞻性选取140例接受血液透析治疗的患者,采用随机抽签法将其分为观察组和对照组,每组70例。对照组予以普通血液透析治疗,观察组则采用高通量血液透析治疗。比较两组临床疗效,治疗前后Hepc、Hb、SF以及CRP水平,治疗前后甲状旁腺激素及血β2微球蛋白水平,以及不良反应发生情况。结果:观察组治疗总有效率为88.57%,高于对照组的72.86%,差异有统计学意义(x^2=5.552,P<0.05);治疗后观察组Hb水平高于对照组,而Hepc、SF和CRP水平均低于对照组,其差异有统计学意义(t=4.221,t=11.303,t=21.793,t=5.903;P<0.05)。治疗后观察组甲状旁腺激素和血β2微球蛋白水平均低于对照组,其差异有统计学意义(t=6.955,t=5.673;P<0.05);观察组不良反应发生率为8.57%,低于对照组的21.43%,差异有统计学意义(x^2=4.538,P<0.05)。结论:高通量血液透析相比普通血液透析具有显著的临床疗效,且有利于Hepc、Hb、SF及CRP水平的改善,并可有效降低不良反应发生风险,具有较好的安全性。 Objective: To study and compare the effects of ordinary hemodialysis and high-throughput hemodialysis on the levels of Hepcidin(Hepc), hemoglobin(Hb), serum ferritin(SF) and c-reactive protein(CRP). Methods: 140 patients who received the treatment of hemodialysis were enrolled in this study, and they were divided into observation group(70 cases) and control group(70 cases) by random lottery. The patients of control group were treated by routine hemodialysis, while those of observation group were treated by high-throughput hemodialysis. And the therapeutic effect between two groups was compared, and the levels of Hepc, Hb, SF, CRP, parathyroid hormone, blood β2 microglobulin and adverse reaction between pre and post treatment also were compared. Results: The total effective rate of treatment of observation group(88.57%) was significantly higher than that of control group(72.86%)(x^2=5.552, P<0.05). After treatment, Hb levels of observation group was significantly higher than that of control group, while the levels of Hepc, SF and CRP were significantly lower than that of control group(t=4.221, t=11.303, t=21.793, t=5.903, P<0.05). After treatment, the levels of thyroid and blood β2 microglobulin of observation group were significantly lower than those of control group(t=6.955, t=5.673, P<0.05), respectively. Besides, the incidence of adverse reactions of observation group(8.57%) was significantly lower than that of control group(21.43%)(x^2=4.538, P<0.05). Conclusion: High-throughput hemodialysis has significant clinically therapeutic effect than that of ordinary hemodialysis, and it is more conducive to improve the levels of Hepc, Hb, SF and CRP, and it can effectively reduce the risk of adverse reactions. It has good safety and it is worthy in clinical application.
