肺癌合并肺部感染患者血清炎性因子水平变化及临床意义 被引量:28

Clinical significance of serum inflammatory factors of lung cancer patients complicated with pulmonary infection

摘要 目的分析肺癌合并肺部感染患者血清炎性因子水平与凝血功能和免疫功能的关系。方法选择医院收治的60例肺癌合并肺部感染患者作为观察组,另选同期收治的未合并肺部感染的肺癌患者60例作为对照组。对比两组血清炎性因子[白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)]、凝血指标[纤维蛋白原(FIB)、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)及凝血酶时间(TT)]、免疫指标(CD3^+、CD4^+、CD4^+/CD8^+、CD8^+及NK细胞),应用Pearson法分析各指标间的相关性。结果观察组血清IL-1β、IL-6及TNF-α均明显高于对照组(P<0.05)。观察组FIB、APTT、PT及TT均明显高于对照组(P<0.05)。观察组的CD3^+、CD4^+、CD4^+/CD8^+及NK细胞均分别低于对照组,CD8^+高于对照组(P<0.05)。患者IL-1β、IL-6及TNF-α分别与PT、CD8+呈正相关(P<0.05),与CD3^+、CD4^+、CD4^+/CD8^+及NK细胞水平呈负相关(P<0.05)。结论肺癌合并肺部感染患者的炎性因子和凝血功能均处于较高水平,免疫功能相对减弱,而炎性因子又与PT和细胞免疫功能密切相关。 OBJECTIVE To detect the levels of serum inflammatory factors of lung cancer patients complicated with pulmonary infection and observe their relationship with coagulation function and immune function. METHODS Totally 60 lung cancer patients complicated with pulmonary infection who were treated in the hospital were chosen as the observation group, meanwhile, 60 lung cancer patients who were not complicated with pulmonary infection were assigned as the control group. The serum inflammatory factors interleukin-1β(IL-1β), interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α), coagulation indexes fibrinogen(FIB), activated partial thromboplastin time(APTT), prothrombin time(PT), thrombin time(TT) and immune indexes(CD3^+, CD4^+, CD4^+/CD8^+, CD8^+ and NK cell) were observed and compared between the two groups. The relationship among the indexes was analyzed by means of Pearson. RESULTS The levels of serum IL-1β, IL-6 and TNF-α of the observation group were significantly higher than those of the control group(P<0.05). The FIB, APTT, PT and TT were significantly higher in the observation group than in the control group(P<0.05). The levels of CD3^+, CD4^+, CD4^+/CD8^+ and NK cell of the observation group were significantly lower than those of the control group, while the CD8+ level of the observation group was significantly higher than that of the control group(P<0.05). The IL-1β, IL-6 and TNF-α were positively correlated with PT and CD8+, respectively(P<0.05) and were negatively correlated with CD3^+, CD4^+, CD4^+/CD8^+ and NK cell(P<0.05). CONCLUSION The levels of inflammatory factors and coagulation indexes of the lung cancer patients complicated with pulmonary infection are high, the immune function is relatively weakened, and the inflammatory factors are closely associated with PT and cellular immune function.
