血清结核杆菌Hsp10对胸腰椎体结核杆菌感染的辅助诊断效果 被引量:3

Effect of serum Mycobacterium tuberculosis Hsp10 on auxiliary diagnosis of thoracolumbar vertebral Mycobacterium tuberculosis infection

摘要 目的探讨血清结核杆菌Hsp10对胸腰椎体结核杆菌感染的诊断效果。方法选取2017年7月-2018年7月住院治疗的疑似胸腰椎体结核患者98例,其中确诊为胸腰椎体结核杆菌感染患者52例为感染组,未感染46例为未感染组,并选取同时健康体检者45例为对照组。所有患者均进行结核菌素的实验检查,结核感染T细胞检测,以及Hsp10含量的检测,比较三种检测方法的有效性和一致性。结果感染组血清结核杆菌Hsp10含量为(125.24±21.57) pg/ml,非感染组为(89.41±20.33) pg/ml,对照组为(86.20±18.87) pg/ml,感染组显著高于非感染组与对照组(P<0.05)。以血清结核杆菌Hsp10含量对胸腰椎体结核杆菌感染作ROC曲线,曲线下面积为0.88,临界值为109.52 pg/ml。血清结核杆菌Hsp10含量检测方法的敏感性80.00%,特异性73.33%;结核菌素的实验敏感性61.54%,特异性31.61%;血清结核杆菌Hsp10含量的检测方法与结核菌素的实验的检测方法具有较高的一致性。结论血清结核杆菌Hsp10含量的检测方法可以作为胸腰椎体结核杆菌感染的有效辅助诊断方法。 OBJECTIVE To explore the effect of serum Mycobacterium tuberculosis Hsp10 on diagnosis of thoracolumbar vertebral Mycobacterium tuberculosis infection. METHODS A total of 98 patients with suspected thoracolumbar vertebral tuberculosis who were hospitalized for treatment from Jul 2017 to Jul 2018 were enrolled in the study, 52 patients who were diagnosed with thoracolumbar vertebral M.tuberculosis infection were assigned as the infection group, and 46 patients who did not have infection were set as the non-infection group, meanwhile, 45 healthy people who received physical examination were chosen as the control group. All of the patients were tested for tuberculin, TB infection T cells and content of Hsp10. The validity and consistency were observed and compared among the three test methods. RESULTS The content of serum M.tuberculosis Hsp10 of the infection group was(125.24±21.57) pg/ml, significantly higher than(89.41±20.33) pg/ml of the non-infection group and(86.20±18.87) pg/ml of the control group(P<0.05). ROC curve was drawn for thoracolumbar vertebral M.tuberculosis infection based on the content of serum M.tuberculosis Hsp10, the area under curve was 0.88, and the critical value was 109.52 pg/ml. The sensitivity of the serum M.tuberculosis Hsp10 test was 80.00%, the tuberculin test 61.54%;the specificity of the serum M.tuberculosis Hsp10 test was 73.33%, the tuberculin test 31.61%;the serum M.tuberculosis Hsp10 test was highly consistent with the tuberculin test. CONCLUSION The serum M.tuberculosis Hsp10 test is effective for auxiliary diagnosis of the thoracolumbar vertebral M.tuberculosis infection.
