用三种术式治疗子宫肌瘤的效果及安全性对比

Effect and safety comparison of three kinds of surgical treatment for uterine fibroids

目的:比较用开腹子宫肌瘤剔除术、腹腔镜下子宫肌瘤剔除术及阴式子宫肌瘤剔除术治疗子宫肌瘤的效果和安全性。方法:将某医院收治的150例子宫肌瘤患者平均分为甲组、乙组和丙组。对甲组患者进行开腹子宫肌瘤剔除术,对乙组患者进行腹腔镜下子宫肌瘤剔除术,对丙组患者进行阴式子宫肌瘤剔除术,然后比较三组患者的各项手术指标。结果:乙组患者和丙组患者术中的出血量均少于甲组患者,其术毕至排气的时间、术毕至下床活动的时间和术后住院的时间均短于甲组患者,P<0.05。三组患者术中剔除肌瘤的个数相比,P>0.05。乙组患者与丙组患者手术的时间、术毕至排气的时间、术毕至下床活动的时间、术后住院的时间及术后并发症的发生率相比,P>0.05。乙组患者和丙组患者术后并发症的发生率均低于甲组患者,P<0.05。结论:与用开腹子宫肌瘤剔除术治疗子宫肌瘤相比,用腹腔镜下子宫肌瘤剔除术与阴式子宫肌瘤剔除术治疗此病均可取得较为理想的效果,可降低患者术后并发症的发生率,缩短其术后康复的时间。

objective: to compare the efficacy and safety of laparoscopic myomectomy, laparoscopic myomectomy and vaginal myomectomy in the treatment of uterine fibroids. Methods: 150 patients with uterine fibroids were divided into group a, group b and group c on average. Open myomectomy was performed on group a patients,laparoscopic myomectomy was performed on group b patients, and vaginal myomectomy was performed on group c patients, and then various surgical indexes of the three groups were compared. Results: the intraoperative blood loss of patients in group b and group c was less than that of patients in group a, and the time from postoperative exhaust to postoperative activity and postoperative hospitalization were shorter than that of patients in group a, P < 0.05. The number of fibroids removed intraoperatively in the three groups was compared, P > 0.05. Compared with group c, group b patients had P > 0.05 in operation time, postoperative time to exhaust gas, postoperative time to get out of bed, postoperative hospitalization time and incidence of postoperative complications. The incidence of postoperative complications in group b and group c was lower than that in group a, P < 0.05. Conclusion: compared with open hysteromyomectomy, laparoscopic hysteromyomectomy and vaginal hysteromyomectomy can achieve better results in the treatment of this disease, which can reduce the incidence of postoperative complications and shorten the postoperative recovery time of patients.
