2010年-2018年10835例髋部骨折病例特点分析

Analysis on the Reason of the Injury and Poison in 10835 Patients with Hip Fracture during 2010 to 2018

目的探讨髋部骨折损伤的流行病特点,为其防治提供依据。方法回顾性收集北京某医院2010年1月1日-2018年12月31日10 835例髋部骨折患者病案首页信息,分别对病例的年龄、性别和季节分布进行描述和分析。结果髋部骨折50岁以上累计占比85.3%,男:女比例1:1.51,且呈年龄递增趋势(χ^2=883.072,P<0.001),其损伤和中毒原因以跌倒和运输事故为主,累计占比高达94.25%,男性以运输事故(61.10%)损伤为主,女性则以跌倒损伤为主(63.71%)。50岁以上跌倒(88.03%)占比高于运输事故(73.52%),且差异有显著性(χ^2=182.330,P<0.001)。跌倒秋冬季高发,运输事故夏秋季高发。结论髋部骨折多见于50岁以上人群,以跌倒和运输损伤为主,且有其独特的性别和季节特点,应区别对待。

Objectives To explore the epidemic characteristics of injury and poisoning in hip fracture, and provide evidence for its prevention and treatment. Methods To retrospect and collect the hip fracture patient case information in a hospital in Beijing, which was about 10835 qualified from January 1 st 2010 to December 31 st 2018.Described and analyzed the age, sex and seasonal distribution of the causes of injury and poisoning, respectively.Results The cumulative ratio of hip fractures above 50 years of age was 85.3 percent, the ratio of men to women was1:1.51, and there was an increasing trend in age by sex ratio(χ^2= 883.072,P<0.001). The main causes of damage and poisoning were falls and transport accidents, which with a cumulative ratio of 94.25 percent. The injury was mainly transport accidents in the male, the percentage was 61.10 percent, and were falls in the female, the percentage was 63.71 percent. In the over 50 years old population, the proportion of falls was 88.03 percent, which was higher than that of transport accidents, that was 73.52 percent, the difference between them was significant(χ^2= 182.330,P<0.001). Fall was high frequency in fall and winter, transport accident was high frequency in summer and fall.Conclusions Hip fractures were more common in people over 50 years of age, and were mainly caused by falls and transport injuries. They had unique gender and seasonal characteristics and should be treated differently.
