为不同年龄嫖客提供服务的低档暗娼的人群特征分析

Characteristics of low-fee female sex workers stratified by the age of clients in key provinces

目的分析向不同年龄嫖客提供服务的低档暗娼的人群特征,尤其是服务≥50岁嫖客的暗娼人群特征,为艾滋病防治工作提供参考。方法通过每半年1次的横断面调查收集了重点省低档暗娼人群的相关资料,内容包括使用问卷调查收集研究对象的一般人口学特征,性交易特点等信息,采集研究对象血样进行艾滋病病毒(HIV)初筛检测及梅毒抗体检测。将现有数据库中6个市县的4次调查数据进行整理合并,剔除重复调查对象形成分析数据库。结果共纳入1 520名研究对象,服务≥50岁嫖客组的低档暗娼平均年龄为(41.18±8.20)岁,小学及以下文化程度占77.2%(345/447),24.5%(110/449)的人户籍所在地集中在调查地区周边所辖县城或乡镇。与服务<50岁嫖客组相比,服务≥50岁嫖客组的暗娼接客地点以出租屋/站街/乡镇集市为主,62.8%(279/444)VS 34.3%(365/1 063);每次性交易价格≤50元比例为82.4%(365/443)VS 36.1%(382/1 057);安全套坚持使用率为43.3%(194/448)VS 62.9%(674/1 071);HIV筛查阳性率为8.7%(39/449)VS 5.8%(62/1 071);梅毒感染率为19.2%(86/449)VS 8.8%(94/1 071)。Logistic回归分析结果表明,低档暗娼人群的年龄、文化程度、户籍所在地、经常接客地点以及每次性交易价格等特征是其服务于50岁及以上嫖客的主要影响因素。结论平均年龄≥40岁,文化程度较低,较多来自性活动地区周边县城或乡镇,主要通过个体出租屋、乡镇赶集点或站街等方式招嫖的低档暗娼是向≥50岁嫖客提供性服务的主要人群,是老年男性感染性病艾滋病的重点高危因素。

Objective To analyze the characteristics of low-fee female sex workers(FSWs), selling sex to the clients at different ages, especially the groups serving clients aged 50 years or above, so as to provide reference for the work of AIDS prevention and control. Methods Information of FSWs in the key provinces were collected through a cross-sectional survey every six months, including the general demographic characteristics, sex trafficking characteristics and blood samples for the HIV screening and syphilis antibody detection. The data of 4 cross-sectional surveys in 6 cities and counties of the control group in the existing database were collated and combined, and the repeated subjects were removed to form an analysis database. Results A total of 1 520 subjects were included in the analysis. The group of FSWs who served≥50 years clients had an average age of 41.18±8.20, with 77.2%(345/447)of primary school education and below, and 24.5%(110/449)living in the counties or towns around the surveyed area. Compared with the group who served the clients < 50 years, those who served clients aged 50 years received clients mainly in the rental houses, streets or town market, by 62.8%(279/444)vs 34.3%(365/1063);the price of each sex transaction was mostly 50 Yuan and below, by 82.4%(365/443)vs 36.1%(382/1 057);consistent condom use rate was lower, by 43.3%(194/448)vs 62.9%(674/1 071);HIV screening positive rate was higher, by 8.7%(39/449)vs 5.8%(62/1 071);syphilis infection rate was higher, by 19.2%(86/449)vs 8.8%(94/1 071). The results of logistic regression analysis showed that age, education level, household registration location, frequent service venues and the sex trade price were the major influence factors of FSWs who served the clients≥50 years. Conclusion FSWs who served the clients≥50 years have the characteristics such as the average age of over 40 years old, lower education level, residents in the counties or towns around the surveyed areas, mainly through the individual rental houses or walking in streets to attract clients, and the low price under 50 Yuan. They are high risk factors of the infection of elderly men.
