血液恶性肿瘤患者创伤后成长状况及其影响因素

Posttraumatic growth of patients with hematologic malignancy and its influencing factors

目的了解血液恶性肿瘤患者创伤后成长状况并探讨其影响因素。方法 2018年5-11月,采用一般资料调查问卷、创伤后成长评定量表、慢性病自我效能量表和中文版心理弹性量表对新疆乌鲁木齐市6所三级甲等医院379名血液恶性肿瘤患者进行问卷调查。结果 379名血液恶性肿瘤患者创伤后成长总分为54.29±12.13;自我效能感总分为5.17±1.49;心理弹性总分为52.11±9.74;创伤后成长总分与自我效能感总分及心理弹性总分均呈正相关(r=0.451、0.503;均P<0.05)。多元线性回归分析显示,学历、家庭人均月收入、自我效能及心理弹性是血液恶性肿瘤患者创伤后成长的主要影响因素(P<0.05)。结论血液恶性肿瘤患者创伤后成长处于中等水平,医护人员应根据患者的不同需求进行个体化管理,改善其自我效能和心理状态,不断提高患者的创伤后成长水平。

Objective To investigate the posttraumatic growth of patients with hematologic malignancy, explore its influencing factors. Methods By using the general data questionnaire, posttraumatic growth scale, chronic disease self-efficacy scale and Chinese version psychological elasticity scale, a questionnaire investigation was conducted among 379 patients with hematologic malignancy in six grade-A tertiary hospitals in Urumqi of Xinjiang from May to November 2018. Results The total score of posttraumatic growth, self-efficacy and psychological elasticity in 379 patients with hematologic malignancy was 54.29 ±12.13, 5.17±1.49 and 52.11±9.74, respectively. The total score of posttraumatic growth was positively correlated with the total score of self-efficacy and the total score of psychological elasticity(r=0.451, 0.503, both P<0.05). The results of multiple linear regression analysis showed that educational background, per capita monthly income of families, self-efficacy and psychological elasticity were the main factors influencing the post-traumatic growth of patients with hematologic malignancy (P<0.05). Conclusion The post-traumatic growth of patients with hematologic malignancy is at a moderate level. Medical staff should carry out individualized management according to the different needs of the patients, improve their self-efficacy and psychological state, and continuously improve the post-traumatic growth level of patients.
