1例重度溃疡性结肠炎患者营养中支持治疗的病例分析

Analysis of Nutrition Support Treatment for a Case of Severe Ulcerative Colitis

1例40岁女性患者,因“反复脓血便2月余,全身皮疹1周”入院,诊断为重度溃疡性结肠炎。入院后停用所有治疗药物,置鼻胃管,予以肠内营养混悬液(SP)1 500 mL/天,鼻饲,治疗一月后,腹泻无缓解,临床药师建议肠内营养制剂调整为:能量密度为1 kcal·mL^-1的肠内营养混悬液(TPF)1 500 mL/天,治疗15天后,每日腹泻减少至6次,每次约50~100 mL,经过26天后,腹泻减少至4次,全身皮疹逐渐消退,皮肤愈合,于治疗第30天出院。

A 40-years-old female patient who was admitted to the hospital because of "repeated bloody purulent stool for more than 2 months and generalized rash for 1 week",and was diagnosed as severe ulcerative colitis.After admission,all therapeutic drugs were discontinued and a nasogastric tube was placed for enteral nutrition suspension SP 1 500 mL per day.Since diarrhea was not relieved after a month of treatment,clinical pharmacists suggested adjusting the enteral nutrition preparation SP to TPF with energy density of 1 kcal·mL^-1 at a dose of 1 500 mL per day.As the suggestion been accepted by clinicians for 15 days,daily diarrhea was reduced to 6 times with about 50-100 mL per time.After 26 days,the diarrhea was reduced to 4 times,the rash gradually subsided and the skin healed.The patient was discharged on the 30th day of treatment.
