肢导联新流程快速诊断室性心动过速的研究 被引量:1

摘要 目的 寻找一种简单快速的诊断室性心动过速(ventricular tachycardia,VT)的方法。方法 对485例宽QRS心动过速患者的体表心电图进行回顾性分析,分别采用Brugada四步法、Vereckei四步法、室速积分法、肢导联新流程进行检验,与心内电生理对照,观察四种方法的敏感性、特异性及准确性。结果室速积分法、肢导联新流程、Brugada四步法、Vereckei四步法诊断VT的准确性分别为98.2%、95.3%、70.4%、68.7%。室速积分法、肢导联新流程对VT的诊断准确性均优于Brugada四步法、Vereckei四步法(P<0.05)。结论 运用肢导联新流程诊断VT准确率较高,且诊断流程简单、易于记忆、便于推广,因此是一种快速诊断VT的新方法。 Objective To look for a simple and rapid method for diagnosing ventricular tachycardia(VT). Methods Retrospective analysis was performed on the surface ECGs of 485 patients with wide QRS tachycardia.Brugada four step method, Vereckei four step method, VT integration method and New process of limb lead were separately applied in the examination.Compared with intracardiac electrophysiology, the sensitivity, specificity and accuracy of the four methods were observed.Results The accuracy rate of VT integration method, new process of limb lead, Brugada four step method and Vereckei four step method were 98.2%, 95.3%, 70.4% and 68.7%, respectively.The accuracy rate of VT integration method and new process of limb lead in diagnosing VT were higher than that of Brugada four step method and Vereckei four step method( P <0.05).Conclusion The accuracy rate of new process of limb lead in diagnosing VT proves to be relatively high.The process is simple, and easy to be remembered and promoted.It is a novel method for making rapid diagnosis of VT.
