术前磁共振对三阴性乳腺癌术后早期复发的评估价值 被引量:7

Preoperative magnetic resonance imaging for early postoperative recurrence of triple negative breast cancer

摘要 目的 探讨术前磁共振对三阴性乳腺癌(triple-negative breast cancer,TNBC)患者术后早期复发的评估价值。方法 收集湖北医药学院附属人民医院2013年1月至2015年1月诊治的TNBC患者的术前磁共振检查资料及临床病理资料,以术后是否复发进行分组随访,分析乳腺癌术后复发的危险因素。结果 共收集TNBC患者93例,其中22例复发,3年复发率为23.7%。与无复发组相比,复发组术前MRI征象中瘤内高信号占比更多(P=0.001)、存在环状强化(P=0.039)、肿瘤形态不规则(P=0.005)以及瘤周水肿征象(P<0.001)。Cox多因素分析结果显示,环状强化(RR=3.008,95%CI:1.137~7.958)、瘤周水肿(RR=5.535,95%CI:1.445~21.194)是TNBC患者术后早期复发的独立危险因素。3年无瘤生存分析显示,环状强化阳性TNBC患者无瘤生存率低于阴性组,分别为57.1%、81.9%(P=0.016);瘤周水肿阳性TNBC患者无瘤生存率低于阴性组,分别为90.7%、56.4%(P=0.000)。结论 TNBC患者术前MRI征象中环状强化、瘤周水肿是术后早期复发的独立危险因素,应引起临床注意。
