腹腔镜技术在胃肠外科急腹症患者治疗中的效果分析 被引量:3

摘要 目的探讨腹腔镜技术在胃肠外科急腹症患者治疗中的效果。方法选取2018年3月-2019年1月该院收治的110例胃肠外科急腹症患者,依据不同的就诊顺序将其分为对照组(n=52)与试验组(n=58),对照组施以常规开腹手术方式治疗,试验组施以腹腔镜手术治疗,观察两组患者的手术时间、切口长度、术中出血量、住院时间、术后体温恢复时间、术后进食时间、术后排气时间、术后下床时间等情况及术后并发症发生情况。结果试验组的手术时间明显较长,住院时间、术后体温恢复时间、术后进食时间、术后排气时间、术后下床时间明显较短,切口长度明显较短,术中出血量明显较少,与对照组相比差异有统计学意义(P<0.05);试验组的切口感染、疼痛、出血、肠粘连等并发症发生率分别为1.72%、3.45%、1.72%、1.72%,明显低于对照组的11.54%、15.38%、13.46%、13.46%,差异有统计学意义(χ^2=4.432、4.727、5.601、5.601,P=0.035、0.030、0.018、0.018);但两组患者的腹胀症状的发生率分别为3.45%与5.77%,差异无统计学意义(χ^2=0.340,P=0.560)。结论腹腔镜技术在胃肠外科急腹症患者治疗中具有显著的临床效果,不仅能够缩短住院时间、术后体温恢复时间、术后进食时间、术后排气时间、术后下床时间,减少术中出血量,同时能够降低并发症发生率,缩短切口长度。 Objective To investigate the effect of laparoscopic technique in the treatment of patients with acute abdomen of gastrointestinal surgery.Methods A total of 110 patients with gastrointestinal surgery acute abdomen admitted to our hospital from March 2018 to January 2019 were divided into control group(n=52)and experimental group(n=58)according to different treatment order.The control group was treated with conventional open surgery.The experimental group was treated with laparoscopic surgery.The operation time,incision length,intraoperative blood loss,hospital stay,postoperative temperature recovery time,postoperative feeding time,and postoperative time were observed,as well as the postoperative exhaust time,postoperative bedtime,etc.and postoperative complications.Results The operation time of the experimental group was significantly longer.The length of hospital stay,postoperative temperature recovery time,postoperative feeding time,postoperative exhaust time,and postoperative bedtime were significantly shorter.The incision length was shorter and the intraoperative blood loss was obvious less,compared with the control group,the difference was statistically significant(P<0.05);the incidence of complication of infection,pain,hemorrhage and intestinal adhesion in the experimental group were 1.72%,3.45%,1.72%,1.72%,respectively,significantly lower than 11.54%,15.38%,13.46%,13.46% of the control group,the difference was statistically significant(χ^2=4.432,4.727,5.601,5.601,P=0.035,0.030,0.018,0.018);however,the incidence of bloating symptoms in the two groups of patients were respectively 3.45% and 5.77%,there was no significant(χ^2=0.340,P=0.560).Conclusion Laparoscopictechnique has significant clinical effects in the treatment of patients with acute abdomen of gastrointestinal surgery.It can not only shorten the length of hospital stay,postoperative temperature recovery time,postoperative feeding time,postoperative exhaust time,postoperative bedtime,and reduce the amount of intraoperative blood loss,the incidence of complications and shorten the length of the incision.
