综合康复护理措施对乳腺癌根治术患者上肢功能和生活质量的影响

Effect of nursing measures on upper limb function and quality of life in patients undergoing radical mastectomy

目的探讨综合康复护理措施对乳腺癌根治术患者上肢功能和生活质量的影响。方法选取2016年1月至2018年12月间陕西省渭南市中心医院收治的接受乳腺癌根治术治疗的184例患者作为研究对象,采用随机数表法分为对照组与研究组,每组92例。对照组患者术后接受常规护理干预,研究组在此基础上应用综合康复护理措施。术后观察3个月,比较两组患者上肢功能的恢复情况。同时采用乳腺癌患者生命质量测定量表评价两组患者术后1周与术后3个月时生活质量的变化。结果两组患者上肢功能各项指标比较,研究组后伸角度、旋内角度和内收肌力均高于对照组,差异均有统计学意义(均P <0. 05)。术后1周时,两组患者情感状况、躯体状况、功能状况、附加关注和社会/家庭状况评分比较,差异无统计学意义(P <0. 05)。术后3个月时,研究组情感状况、躯体状况、功能状况、附加关注和社会/家庭状况评分均高于对照组,差异均有统计学意义(均P <0. 05)。结论综合康复护理措施可以有效改善乳腺癌根治术患者上肢功能与生活质量,值得临床推广。

Objective To analyze the effect of nursing measures on upper limb function and quality of life in patients undergoing radical mastectomy. Methods A total of 184 patients who underwent radical mastectomy at Weinan Central Hospital from January 2016 to December 2018 were selected as the research subjects. They were divided into a study group and a control group with 92 patients in each group by random number table method. Patients in the study group received comprehensive rehabilitation nursing besides routine nursing,while patients in the control group received routine nursing intervention after operation. They were followed up for three months. The recovery of upper limb function was compared between the two groups. Meanwhile,the quality of life was evaluated by functional assessment of cancer therapy-breast( FACT-B) at one week after the operation and three months after the operation. Result In the comparison of the indexes of upper limb function between the two groups,the extension angle,rotation angle and adductor muscle strength were higher in the study group than in the control group( all P < 0. 05). There was no significant difference in emotional status,physical status,functional status,additional attention and social/family status score between the two groups at 1 week after the operation( all P < 0. 05). At 3 months after the operation,the emotional status,physical status,functional status,additional attention and social/family status score were higher in the study group than in the control group( all P < 0. 05). Conclusion Comprehensive rehabilitation nursing measures can effectively improve the upper limb function and quality of life in patients undergoing radical mastectomy,which is worthy of clinical promotion.
