临床药师参与1例肿瘤患者导管相关血流感染的药学实践

Pharmacological Practice of Clinical Pharmacists Involved in a Catheter Related Blood Stream Infection in a Cancer Patient

探讨临床药师参与肿瘤患者外周中心静脉置管相关性血流感染药物治疗方案的制定以及提供药学服务的方法。临床药师参与1例导管相关血流感染肿瘤患者的抗感染治疗过程,对治疗期间患者感染相关因素、可能病原菌、抗感染治疗方案选择及疗效进行分析,优化治疗方案。通过临床药师参与调整抗感染治疗方案后,患者病情较前明显好转,转至下级医院降阶梯继续抗感染治疗。临床药师通过参与临床实践,为临床医生提供循证医学证据,为患者提供个体化抗感染治疗方案,在提高治疗效果的同时减少不良反应的发生,实现合理用药。

To explore the methods for drug therapy regimen and pharmaceutical service of clinical pharmacist for catheter related blood stream infection in cancer patients.Clinical pharmacist participated in an anti-infective treatment process of a cancer patient with catheter related bloodstream infection,and analyzed the related factors,possible pathogens,anti-infective treatment options and curative effects during the treatment,and finally optimized the treatment plan.After the clinical pharmacist participated in the adjustment of the anti-infective treatment plan,the patient’s condition was improved significantly,and the patient went to the lower level hospital to continue the anti-infective treatment.Through the participation in clinical practice,clinical pharmacists provide evidence-based medical evidence for clinicians and individualized anti-infective treatment programs for patients,therefore,increase the effectiveness of treatment and reduce the incidence of adverse reactions to achieve rational use of medicines.
