临床药师参与1例耐万古霉素肠球菌重症患者的抗感染治疗,协助医师制定和调整抗感染治疗方案,对抗感染治疗的疗效进行监护,防止患者在治疗过程中出现抗菌药物相关腹泻。以患者在治疗过程出现的2次药品不良反应为契机,强化了医护人员上报药品不良反应的意识。临床药师以参与者的身份加入治疗团队中,提高药物治疗的有效性和安全性,为临床合理用药提供支持。

Clinical pharmacist participated in the diagnosis and treatment of anti-infective treatment for a patient with vancomycin-resistant enterococcus.Assisted physicians in formulating and adjusting anti-infective treatment programs,monitored the efficacy of anti-infective therapy,and prevented the patient from developing antibiotic-related diarrhea in the course of treatment.The clinical pharmacist taken advantage of the opportunity of two adverse drug reactions to strengthen the awareness of medical staff to report adverse drug reactions.After clinical pharmacists join the treatment team,they can improve the effectiveness and safety of drug treatment and provide support for rational drug use in clinic.
