同型半胱氨酸联合D-二聚体水平检测对肿瘤患者血栓形成的预测价值

Predictive value of homocysteine combined with D-dimer on thrombosis in tumors

目的:探讨同型半胱氨酸联合D-二聚体对肿瘤患者血栓形成的预测价值。方法:选取西安交通大学第一附属医院肿瘤科2017年1月至2018年5月收治的肿瘤患者1 214例为研究对象。按照D-二聚体(D-D)、同型半胱氨酸(Hcy)水平将肿瘤患者分为三组:低危组:D-D、Hcy均阴性;中危组:D-D或Hcy阳性;高危组:D-D、Hcy均阳性。以住院期间、出院1个月、3个月内任一次下肢静脉超声发现静脉血栓为观察终点。比较三组在肿瘤患者血栓形成中的预测价值。结果:D-D在不同肿瘤中检测比例无统计学差异;与其他肿瘤比较,Hcy在原发性肝癌组中检出率明显增加(P<0.05);与高危组比较,低危组及中危组静脉血栓发生率明显减少(P<0.05)。结论:同型半胱氨酸联合D-二聚体对肿瘤患者血栓形成具有明确的预测价值。

Objective:To investigate the predictive value of homocysteine(Hcy) combined with D-dimer(D-D)on thrombosis in tumors patients.Methods:1 214 patients with malignant tumors,admitted to the Oncology Department of the First Affiliated Hospital of Xi’an Jiaotong University from January 2017 to May 2018,were selected.The patients were divided into three groups according to the level of D-D and Hcy:Low-risk group:D-D and Hcy were negative,medium-risk group:D-D or Hcy positive,high-risk group:D-D and Hcy were positive.The end point of observation was venous thrombosis detected in lower extremity veins by ultrasound during hospitalization,one month and three months after discharge,and compare the predictive value of three groups on thrombosis in cancer patients was compared. Results: There was no statistical difference in the proportion of D-D in different tumors. Compared with other tumors group,the detection rate of Hcy in primary hepatocellular carcinoma group increased significantly( P < 0. 05). Compared with high-risk group,the incidence of venous thrombosis decreased significantly in low-risk group and medium-risk group( P < 0. 05). Conclusion: Homocysteine combined with D-dimer help to predict the thrombosis in cancer patients.
