护理营养干预对食管癌患者术后营养状况的影响效果分析 被引量:3

摘要 目的分析食管癌患者手术后给予营养护理干预的临床价值,并分析其对患者手术后的机体营养状况。方法方便选择该院2015年8月-2017年12月期间收治的82例食管癌患者作为该次研究对象,所有患者均行手术治疗,手术后1周对患者的体质量指数(BMI)、上臂中围(MAC)、上臂肌围(AMC)、肱三头肌皮褶厚度(TSF)、总淋巴细胞计数(TLC)、血清白蛋白(ALB)进行监测,同时给予患者营养支持护理干预,比较不同时间段的营养状况变化。结果手术后1周食管癌患者营养不良发生率为87.80%,与手术后2周(65.85%)比较差异有统计学意义(χ^2=11.097 7,P<0.05),食管癌患者手术后2周的体质量指数(21.69±1.42)kg/m^2、上臂肌围(22.85±1.54)cm、肱三头肌皮褶厚度(12.83±1.24)mm、上臂中围(24.19±1.16)cm、总淋巴细胞计数(1.61±1.02)×10^9/L以及血清白蛋白(35.38±1.27)g/L水平显著高于手术后1周的营养指标水平,其中上升幅度最大的就是血清白蛋白,由(32.29±3.07)g/L上升高(35.38±1.2)g/L,差异有统计学意义(P<0.05)。结论食管癌患者手术前1周有显著的营养不良状况,而在手术后1周表现更为明显,患者手术后2周到出:时有所降低,但是仍旧存在营养不良状况,此种情况,护理人员应该对患者实施有效的营养支持护理干预措施,以此促进患者康复。 Objective To analyze the clinical value of nutritional nursing intervention in patients with esophageal cancer after operation, and to analyze the nutritional status of patients after surgery. Methods Convenient select eighty-two patients with esophageal cancer admitted to our hospital from August 2015 to December 2017 were enrolled. All patients underwent surgery. The body mass index(BMI) and upper arm Peripheral(MAC), upper arm muscle circumference(AMC), triceps skinfold thickness(TSF), total lymphocyte count(TLC), serum albumin(ALB) were measured one week after surgery, and nutritional support interventions were given to patients. Changes in nutritional status over time was compared. Results The incidence of malnutrition in patients with esophageal cancer was 87.80% 1 week after surgery, which was significantly different from 2 weeks after surgery(65.85%)(χ^2=11.097 7, P<0.05). Body mass index of patients with esophageal cancer 2 weeks after surgery(21.69±1.42)kg/m^2, upper arm muscle circumference(22.85±1.54)cm, triceps skinfold thickness(12.83±1.24)mm,upper arm middle circumference(24.19±1.16)cm, total lymphocyte count(1.61±1.02)×10^9/L and serum albumin(35.38±1.27)g/L were significantly higher than those at 1 week after surgery. The highest increase was serum albumin, which was increased by(35.38±1.27)g/L from(32.29±3.07)g/L. the difference was significant, and there was statistical significance(P<0.05). Conclusion Patients with esophageal cancer have significant malnutrition 1 week before surgery, and the performance is more obvious one week after surgery. The patient has decreased after 2 weeks of surgery, but there is still malnutrition. In this case, the nursing staff of effective nutrition support care interventions should be implemented on patients to promote patient recovery.
