股骨近端空心锁定板内固定治疗股骨颈骨折的临床效果

Clinical Effect of Internal Fixation of the Proximal Femoral Hollow Locking Plate for the Treatment of Femoral Neck Fracture

目的探讨股骨近端空心锁定板内固定治疗股骨颈骨折的疗效。方法选取2016年1月—2018年6月该院收治的90例股骨颈骨折患者作为研究对象,随机分为两组,每组45例。对照组选择空心钉内固定,观察组选择股骨近端空心锁定板内固定。分析手术成功率、手术执行的时间、总出血量、可负重的时间、平均住院时间;治疗前后患者髋关节功能Harris评分;并发症。结果两组手术成功率比较,差异无统计学意义(χ^2=0.000,P>0.05)。两组均是100.00%成功。治疗前两组髋关节功能Harris评分比较,差异无统计学意义(t=0.134,P>0.05);治疗后观察组髋关节功能Harris评分高于对照组,差异有统计学意义(t=5.214,P<0.05)。观察组手术执行的时间、可负重的时间、平均住院时间短于对照组,总出血量少于对照组,差异有统计学意义(t=7.991、5.202、6.325、5.715,P<0.05)。观察组并发症低于对照组,差异有统计学意义(χ^2=7.923,P<0.05),观察组并发症中,感染有1例,髋内翻有1例,下肢深静脉血栓有1例,总发生是6.67%。对照组并发症中,感染有4例,髋内翻有2例,下肢深静脉血栓有3例,总发生是20.00%。结论股骨颈骨折患者实施股骨近端空心锁定板内固定可获得较好效果,可有效缩短手术的时间和住院时间,减少并发症,促使患者的髋关节功能得到改善。

Objective To investigate the effect of internal fixation of the proximal femoral locking plate in the treatment of femoral neck fractures. Methods Ninety patients with femoral neck fractures admitted to our hospital from January 2016 to June 2018 were randomly divided into two groups, 45 cases in each group. The control group was treated with cannulated internal fixation, and the observation group was selected for internal fixation of the proximal femoral hollow locking plate. The success rate of the operation was analyzed;the time of operation, the total amount of bleeding, the time of weight-bearing, the average length of hospital stay;the Harris score of hip function before and after treatment;complications were compared. Results There was no statistically significant difference in the success rate between the two groups (χ^2=0.000, P>0.05). Both groups were 100.00% successful. There was no statistically significant difference in hip function Harris score between the two groups before treatment (t=0.134, P>0.05). After treatment, the hip function Harris score was higher in the observation group than in the control group (t=5.214, P<0.05). The operation time, weight-bearing time, and average hospitalization time of the observation group were shorter than that of the control group. The total amount of bleeding was less than that of the control group, respectively, and the difference was statistically significant(t=7.991、5.202、6.325、5.715,P<0.05). The complication of the observation group was lower than that of the control group, the difference was statistically significant (χ^2=7.923, P<0.05). In the observation group, there were 1 case of infection, 1 case of hip varus, and 1 case of deep venous thrombosis of the lower extremity. The total occurrence was 6.67%. In the control group, there were 4 cases of infection, 2 cases of hip varus, and 3 cases of deep venous thrombosis of the lower extremity, the total incidence was 20.00%. Conclusion Patients with femoral neck fractures can achieve better results by internal fixation of the proximal femoral locking plate, which can effectively shorten the operation time and hospitalization time, reduce complications, and improve the hip function of patients.
