ICU重症并发上消化道出血的治疗及预后评定

Treatment and Prognostic Evaluation of Severe Upper Gastrointestinal Bleeding in ICU

目的探讨ICU重症并发上消化道出血的治疗及预后评定。方法采集本院80例2015年6月-2018年6月ICU重症并发上消化道出血患者。随机数字表分组,常规治疗组采纳常规药物治疗,对症支持治疗组则采纳常规药物加上对症支持治疗。比较常规治疗组、对症支持治疗组疗效;生命体征稳定的时间、上消化道出血停止时间;治疗前后患者血红蛋白、重症APACHEⅡ评分;死亡率。结果对症支持治疗组的总有效率高于常规治疗组差异有统计学意义(P <0.05);生命体征稳定的时间、上消化道出血停止时间少于常规治疗组,血红蛋白水平指标和重症APACHEⅡ评分优于常规治疗组,差异有统计学意义(P <0.05);对症支持治疗组死亡率低于常规治疗组,差异有统计学意义(P <0.05)。结论常规药物加上对症支持治疗ICU重症并发上消化道出血的效果理想。

Objective To investigate the treatment and prognosis of severe upper gastrointestinal bleeding in ICU. Methods 80 patients with severe upper gastrointestinal bleeding in ICU from June 2015 to June 2018 were collected. Randomized number table grouping, routine treatment group adopt routine drug treatment, symptomatic support group adopt routine drug plus symptomatic support treatment. Comparing the curative effect of routine treatment group and symptomatic support treatment group, the time of vital signs stabilization, the stop time of upper gastrointestinal bleeding, the scores of hemoglobin and severe APACHE Ⅱ before and after treatment, and the mortality rate. Results The total effective rate of symptomatic support group was higher than that of routine treatment group (P < 0.05);the time of vital signs stabilization and upper gastrointestinal bleeding stopping was less than that of routine treatment group;the hemoglobin level index and severe APACHE Ⅱ score were better than that of routine treatment group (P < 0.05). The mortality in the symptomatic support group was lower than that in the conventional treatment group (P < 0.05). Conclusion Conventional drugs plus symptomatic support are effective in the treatment of severe upper gastrointestinal bleeding in ICU.
