腹腔镜和开腹直肠癌根治术对患者肠功能的影响

Effects of Laparoscopic and Open Surgery on Intestinal Function in Patients

目的探讨腹腔镜与开腹直肠癌根治术对患者肠功能恢复的影响。方法选择2016年3月—2018年6月84例直肠癌患者,随机分组。开腹直肠根治术组选择开腹直肠癌根治术,腹腔镜根治术组选择腹腔镜直肠癌根治术。分析相关围术期的指标如肠鸣音恢复时间、排气时间、术后平均吗啡用量、手术出血总量和住院时间;治疗前后患者胃肠功能胃泌素和胃动素指标;并发症。结果腹腔镜根治术组胃肠功能胃泌素和胃动素指标、相关围术期的指标如肠鸣音恢复时间、排气时间、术后平均吗啡用量、手术出血总量和住院时间、并发症和开腹直肠根治术组比较有优势,差异有统计学意义(P <0.05)。结论直肠癌患者实施腹腔镜直肠癌根治术可获得较好效果,可减轻对胃肠的不良刺激,加速术后肠功能的恢复。

Objective To investigate the effect of laparoscopic and open radical resection of rectal cancer on the recovery of intestinal function. Methods 84 patients with rectal cancer from March 2016 to June 2018 were randomly divided into two groups. Open rectal radical operation group chose open rectal cancer radical operation, laparoscopic radical operation group chose laparoscopic rectal cancer radical operation. Relevant perioperative indicators such as recovery time of bowel sounds, exhaust time, average morphine dosage after operation, total amount of bleeding and hospital stay were analyzed;gastrin and motilin indicators of gastrointestinal function before and after treatment;complications. Results Laparoscopic radical operation group had advantages in gastrointestinal function, motilin and related perioperative indicators such as recovery time of bowel sounds, exhaust time, average morphine dosage after operation, total amount of bleeding and hospital stay, complications and open rectal operation group (P < 0.05). Conclusion Laparoscopic radical resection of rectal cancer can achieve better results, alleviate the adverse stimulation to gastrointestinal tract and accelerate the recovery of intestinal function after operation.
