无创呼吸机治疗慢性阻塞性肺疾病合并II型呼吸衰竭疗效 被引量:8

Non-invasive Ventilator for the Treatment of Chronic Obstructive Pulmonary Disease with Type Ⅱ Respiratory Failure

摘要 目的探讨无创呼吸机在治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭的临床效果。方法选取本院2017年2月-2018年2月期间内接收的116例慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者,随机分为对照组与观察组,每组58例。分别接受常规治疗、常规治疗联合无创呼吸机治疗。对比治疗效果、治疗后肺功能、血气各指标水平。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组治疗后FVC、FEV1、PaO2水平高于对照组,PaCO2水平低于对照组,差异有统计学意义(P<0.05)。两组患者在治疗期间无明显的不良反应,仅观察组患者出现了1例腹部胀气,但经过相应的对症治疗后明显缓解,两组未出现呼吸功能衰竭及死亡病例。结论慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭患者经无创呼吸机治疗后可以使肺功能显著改善,血气指标逐渐恢复至正常水平。 Objective To investigate the clinical effect of noninvasive ventilator in the treatment of chronic obstructive pulmonary disease with type Ⅱ respiratory failure. Methods 116 patients with chronic obstructive pulmonary disease and type Ⅱ respiratory failure who were received from February 2017 to February 2018 were randomly divided into control group and observation group, with 58 cases in each group. Only conventional treatment, conventional treatment combined with non-invasive ventilator treatment were accepted. Compare the therapeutic effect, post-treatment lung function, blood gas index levels. Results The total effective rate of the observation group was higher than that of the control group (P < 0.05). After treatment, the levels of FVC, FEV1 and PaO2 in the observation group were higher than those in the control group, while the levels of PaCO2 in the observation group were lower than those in the control group (P < 0.05). There were no obvious adverse reactions in the two groups during the treatment. Only one patient in the observation group had abdominal flatulence, but it was relieved after corresponding symptomatic treatment. There were no respiratory failure and death cases in the two groups.Conclusion The pulmonary function of COPD patients with type Ⅱ respiratory failure can be significantly improved after non-invasive ventilator treatment, and the blood gas index gradually returns to normal level.
