Ⅱb期宫颈癌术前同步放化疗24例疗效分析

Ⅱb Stage Cervical Cancer Preoperative Chemoradiation in 24 Patients with Curative Effect Analysis

目的研究Ⅱb期宫颈癌术前同步放化疗的效果。方法以2016年5月-2018年4月于武汉大学人民医院接诊的Ⅱb期宫颈癌患者48例为研究对象,将之采用随机数表法分成实验组和对照组,每组各有24例。实验组实行手术前同步放化疗治疗,对照组术前实行新辅助化疗。分析两组的近期疗效,比较子宫旁浸润发生率等指标。结果实验组近期疗效的总有效率为91.67%,比对照组的66.67%高,差异有统计学意义(P<0.05)。实验组的子宫旁浸润发生率为12.5%、盆腔淋巴结转移率为8.33%,比对照组的37.5%、33.33%低,差异有统计学意义(P<0.05)。实验组的不良反应发生情况、手术时间、膀胱功能恢复时间和术中出血量同对照组比较,差异不存在统计学意义(P>0.05)。结论采取术前同步放化疗法对Ⅱb期宫颈癌病患进行施治,可显著提高其近期疗效,促进病情恢复,降低子宫旁浸润以及盆腔淋巴结转移发生率。

Objective To study the effect of preoperative concurrent radiotherapy and chemotherapy for stage Ⅱ B cervical cancer. Methods Forty-eight patients with stage Ⅱ B cervical cancer who were admitted to the People's Hospital of Wuhan University from May 2016 to April 2018 were divided into experimental group and control group by random number table method. Each group had 24 cases. The experimental group was treated with concurrent radiotherapy and chemotherapy, while the B group was treated with neoadjuvant chemotherapy before operation. The short-term efficacy of the two groups was analyzed and the incidence of parametrial infiltration was compared. Results The total effective rate was 91.67% in the experimental group, which was higher than 66.67% in the control group (P < 0.05). The incidence of parametrial infiltration and pelvic lymph node metastasis was 12.5% and 8.33% in the experimental group, which was lower than 37.5% and 33.33% in the control group (P < 0.05). There was no significant difference in adverse reactions, operation time, bladder function recovery time and intraoperative bleeding between the experimental group and the control group (P > 0.05).Conclusion Preoperative concurrent chemoradiotherapy for stage Ⅱ B cervical cancer can significantly improve its short-term efficacy, promote recovery, reduce the incidence of parametrial infiltration and pelvic lymph node metastasis.
