脑梗死患者血浆致动脉硬化指数与颈动脉粥样硬化的相关性研究 被引量:23

摘要 目的探讨脑梗死(cerebral infarction,CI)患者血浆致动脉硬化指数(atherogenic index of plasma,AIP)与颈动脉粥样硬化的关系。方法收集2016年1月-2018年12月在锦州医科大学附属第一医院神经内科住院治疗的发病48 h内的CI患者为研究组,经头部CT和/或MRI检查确诊;选取同期我院体检中心健康体检者作为对照组。两组性别和年龄相匹配。采用多普勒超声检测颈动脉内膜中层厚度(carotid artery intima-media thickness,CIMT);同时检测血清甘油三酯(triglycerides,TG)、总胆固醇(total cholesterol,TC)、高密度脂蛋白胆固醇(highdensity lipoprotein cholesterol,HDL-C)和低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C),根据公式lg(TG/HDL-C)计算AIP,进行统计学分析。结果(1)研究组中的CIMT增厚率明显高于对照组(P <0. 05);(2)研究组内CIMT增厚和CIMT正常两亚组间在TG、TC、LDL-C和HDL-C方面的比较有统计学意义(P <0. 05);(3)研究组中AIP和LDL-C与CIMT呈正相关(P <0. 05),AIP与颈动脉内斑块的稳定性呈负相关(P <0. 05)。结论AIP与CIMT和管腔内斑块稳定性密切相关,能够评估颈动脉粥样硬化发生的风险及严重程度。 Objective In order to investigate the relationship between atherogenic index of plasma( AIP) and carotid atherosclerosis in patients with cerebral infarction( CI). Methods CI patients hospitalized in the Department of Neurology of the First Affiliated Hospital of Jinzhou Medical University within 48 hours from December 2016 to December 2018 were collected as the study group,and confirmed by head CT and/or MRI. Healthy subjects in the physical examination center of our hospital during the same period were selected as the control group. The two groups matched gender and age. Carotid artery intima-media thickness( CIMT) was determined by doppler ultrasound. While detecting serum triglycerides( TG),total cholesterol( TC),high-density lipoprotein cholesterol( HDL-C) and low density lipoprotein cholesterol( LDL-C),AIP,lg( TG/HDL-C),was calculated for statistical processing. Results( 1) CIMT thickening rate in the study group was significantly higher than that in the control group( P < 0. 05).( 2) There were significant differences in TG,TC,LDL-C and HDL-C between the two subgroups in the study group( P < 0. 05).( 3) There was a positive correlation between AIP and LDL-C and CIMT in the study group( P < 0. 05),and AIP was negatively correlated with carotidplaque stability( P <0. 05). Conclusion AIP is closely related to CIMT and intraluminal plaque stability,and can assess the risk and severity of carotid atherosclerosis.
