气压弹道式体外冲击波治疗鹅足滑囊炎疗效观察 被引量:7

摘要 目的观察气压弹道式体外冲击波治疗鹅足滑囊炎的临床效果。方法选择2014年7月至2018年6月鹤壁市人民医院收治的56例鹅足滑囊炎患者为研究对象。根据治疗方法将患者分为观察组(n=29)和对照组(n=27),观察组患者给予气压弹道式体外冲击波治疗,对照组患者给予双氯芬酸二乙胺乳胶剂及红外线理疗仪治疗。治疗前及治疗1、4周,采用视觉模拟评分法(VAS)评估患者疼痛情况,采用纽约特种外科医院(HSS)评分系统评估患者膝关节功能;治疗4周后评价2组患者的临床疗效。结果2组患者治疗前VAS评分比较差异无统计学意义(t=0.161,P>0.05);观察组患者治疗1、4周后VAS评分显著低于治疗前(t=5.875、6.742,P<0.05),治疗4周后VAS评分显著低于治疗1周(t=4.685,P<0.05)。对照组患者治疗1、4周VAS评分显著低于治疗前(t=3.631、3.762,P<0.05),但治疗1、4周时VAS评分比较差异无统计学意义(t=0.261,P>0.05)。治疗1、4周,观察组患者VAS评分显著低于对照组(t=4.675、4.542,P<0.05)。2组患者治疗前HSS评分比较差异无统计学意义(t=0.121,P>0.05);观察组患者治疗1、4周后HSS评分显著高于治疗前(t=7.365、8.658,P<0.05),治疗4周后HSS评分显著高于治疗1周(t=5.245,P<0.05)。对照组患者治疗1、4周后HSS评分显著高于治疗前(t=5.735、6.157,P<0.05),但治疗1、4周时HSS评分比较差异无统计学意义(t=0.215,P<0.05)。治疗1、4周,观察组患者HSS评分显著高于对照组(t=4.357、5.558,P<0.05)。治疗4周,观察组患者痊愈率为75.9%(22/29),总有效率为93.1%(27/29);对照组患者痊愈率为29.6%(8/27),总有效率为74.1%(20/27);观察组患者痊愈率及总有效率显著高于对照组(χ^2=9.867、4.658,P<0.05)。结论气压弹道式体外冲击波可以显著缓解鹅足滑囊炎患者疼痛,改善患者膝关节功能。 Objective To investigate the clinical effect of pneumatic ballistic extracorporeal shock wave in treatment of pes anserinus bursitis.Methods Fifty-six patients with pes anserinus bursitis admitted to the People′s Hospital of Hebi City from July 2014 to June 2018 were selected as the study subjects,and they were divided into observation group(n=29)and control group(n=27)according to the treatment method.The patients in the observation group were treated with pneumatic ballistic extracorporeal shock wave,while the patients in the control group were treated with diclofenac diethylamine latex and infrared physiotherapy.The pain of patients was evaluated by visual analogue scale(VAS),and the knee joint function was evaluated by Hospital for special surgery(HSS)scoring system before treatment and after one and four weeks of treatment.The clinical efficacy of patients in the two groups was evaluated after four weeks of treatment.Results There was no significant difference in VAS score between the two groups before treatment(t=0.161,P>0.05).The VAS score after one and four weeks of treatment was significantly lower than that before treatment(t=5.875,6.742;P<0.05),and the VAS score of patients after four weeks of treatment was significantly lower than that after one week of treatment in the observation group(t=4.685,P<0.05).The VAS score at one and four weeks of treatment was significantly lower than that before treatment in the control group(t=3.631,3.762;P<0.05),but there was no significant difference in VAS score between one and four weeks of treatment(t=0.261,P>0.05).After one and four weeks of treatment,the VAS score in the observation group was significantly lower than that in the control group(t=4.675,4.542;P<0.05).There was no significant difference in HSS score between the two groups before treatment(t=0.121,P>0.05).The HSS score after one and four weeks of treatment was significantly higher than that before treatment(t=7.365,8.658;P<0.05),and the HSS score after four weeks of treatment was significantly higher than that after one week of treatment in the observation group(t=5.245,P<0.05).The HSS score after one and four weeks of treatment was significantly higher than that before treatment(t=5.735,6.157;P<0.05),but there was no significant difference in HSS score between one and four weeks of treatment in the control group(t=0.215,P<0.05).After one and four weeks of treatment,the HSS score in the observation group was significantly higher than that in the control group(t=4.357,5.558;P<0.05).After four weeks of treatment,the healing rate and total effective rate of patients in the observation group was 75.9%(22/29)and 93.1%(27/29),respectively;the healing rate and total effective rate of patients in the control group was 29.6%(8/27)and 74.1%(20/27),respectively.The healing rate and total effective rate of patients in the observation group were significantly higher than those in the control group(χ^2=9.867,4.658;P<0.05).Conclusion The pneumatic ballistic extracorporeal shock wave can significantly relieve the pain and improve the knee joint function of patients with pes anserinus bursitis.
