临床药师参与社区家庭医生服务模式的探讨

Discussion on the service model of clinical pharmacists participating in community family doctors

目的探讨临床药师参与社区家庭医生服务模式及其实践价值。方法选取颛桥社区卫生服务中心社区居民111名进行问卷调查。调查问卷内容包括被调查人员对药品常识的知晓情况、对家庭常备药物的管理情况以及希望获得的药学服务及提供药学服务的方式等。调查过程中,调查人员只能对被调查人员进行相关问卷的解释,不允许对被调查人员进行诱导。结果共发放111份问卷,回收111份有效问卷,回收率为100%。调查结果显示,社区居民对药品说明理解能力较差,需要进行相关知识宣教;36%、27%的社区居民对药品的摆放、储存条件不了解,可能造成降低药效、缩短存放时间等情况。社区居民对药品的服药方法(63.9%)、服药注意事项(48.6%)、常见药品的不良反应处理方法(48.6%)相关知识需求较大。结论社区药师对辖区居民开展药品的不良反应介绍、服药方法、药品的储存等相关知识及用药指导十分必要。

Objective To explore the service model of clinical pharmacists participating in community family doctors and its practical value.Methods 111 community residents in Zhuanqiao Community Health Service Center were selected to conduct a questionnaire survey.Questionnaires included the knowledge of the respondents about drug knowledge,the management of household standing drugs,the pharmaceutical care they hoped to obtain and the way to provide pharmaceutical care.In the process of investigation,investigators can only explain the relevant questionnaires to the respondents,and they are not allowed to induce the respondents.Results 111 questionnaires were sent out and 111 valid questionnaires were collected,with a recovery rate of 100%.The survey results show that the community residents have poor understanding of drug instructions and need to educate them about relevant knowledge.36%and 27%of the community residents do not know about the placement and storage conditions of drugs,which may lead to the reduction of drug efficacy and shortening of storage time.Community residents need more knowledge about the methods of taking drugs(63.9%),matters needing attention(48.6%)and the methods of dealing with adverse reactions of common drugs(48.6%).Conclusion It is necessary for community pharmacists to introduce adverse drug reactions,medication methods,drug storage and other related knowledge and medication guidance.
