摘要
目的分析肺炎支原体肺炎患儿的免疫指标监测的临床意义。方法选择2017年1月~2017年12月新疆医科大学第二附属医院儿科住院肺炎支原体肺炎患儿240例,根据患儿病情的严重程度分为非重症组与重症组各120例,两组患儿均于入院时、出院前晨起空腹采血,使用罗氏cobas702仪器检测血清IgM、IgG、IgA、C3、C4等免疫球蛋白指标,同时使用SEMIBIO仪器检测全血CD4^+、CD8^+及CD4^+/CD8^+等T淋巴细胞亚群指标,对比两组患儿不同时间段各项指标的检测结果。结果重症组入院时的IgM、IgG、IgA、C3、C4、CD8^+均高于非重症组,CD4^+、CD4^+/CD8^+均低于非重症组,差异有统计学意义(P<0.05);重症组出院前的IgG、C4均高于非重症组,差异有统计学意义(P<0.05)。非重症组与重症组出院前的IgM、IgG、IgA、C3、CD4^+、CD4^+/CD8^+均优于本组入院时,差异有统计学意义(P<0.05)。结论对肺炎支原体肺炎患儿进行免疫指标的监测,有助于病情严重程度及预后的准确判断。
Objective To analyze the clinical significance of monitoring immune indexes in children with Mycoplasma pneumoniae pneumonia.Methods 240 children with Mycoplasma pneumoniae pneumonia in different conditions were selected from January 2017 to December 2017 in the pediatric department of the Second Affiliated Hospital of Xinjiang Medical University.According to the severity of the disease,they were divided into non-severe group and severe group,120 cases in each group.Blood samples were taken fasting from admission,treatment and morning before discharge.Serum IgM,IgG,IgA,C3 and C4 were detected by Roche Cobas 702 instrument.T lymphocyte subsets such as CD4^+,CD8^+and CD4^+/CD8^+in whole blood were detected by SEMIBIO.The results of the two groups were compared at different time.Results The levels of IgM,IgG,IgA,C3,C4 and CD8+in the severe group were higher than those in the non-severe group,and the levels of CD4^+,CD4^+/CD8^+were lower than those in the non-severe group,the difference was significant(P<0.05);the levels of IgG and C4 in the severe group were higher than those in the non-severe group before discharge,the difference was significant(P<0.05).Before discharge,IgM,IgG,IgA,C3,CD4^+,CD4^+/CD8^+in non-severe group and severe group were better than those in this group,the difference was significant(P<0.05).Conclusion Monitoring the immune indices of children with Mycoplasma pneumoniae pneumonia is helpful to judge the severity of the disease,the therapeutic effect and the prognosis accurately.
