超声引导下经直肠前列腺穿刺诊断前列腺癌的价值

The value of ultrasound-guided transrectal prostate puncture in the diagnosis of prostate cancer

目的探析在超声引导下进行经直肠前列腺穿刺活检对于前列腺癌的诊断价值.方法于湖北省麻城市人民医院功能科选取疑似前列腺癌患者102例进行分析,纳入患者均采用超声引导下经直肠前列腺穿刺活检术进行诊断.以手术病理诊断结果为金标准,分析在超声引导下进行经直肠前列腺穿刺活检的诊断准确性.结果 102例患者中有54例(52.94%)经穿刺活检诊断为前列腺癌,其余48例为良性疾病(47.06%),与手术病理诊断结果比较,其诊断准确率为96.08%(98/102),灵敏度为94.64%(53/56),特异度为97.83%(45/46).术后部分患者出现疼痛、血尿、发热等并发症,并于数天后自然恢复.结论超声引导下进行经直肠前列腺穿刺活检对前列腺癌进行诊断的灵敏度与特异度较高,可提高前列腺癌的术前诊断准确性,虽有一定并发症出现,但程度轻微,可自行恢复,具有较高的临床应用价值.

Objective To explore the value of transrectal prostate biopsy guided by ultrasound in the diagnosis of prostate cancer. Methods 102 suspected prostate cancer patients were selected from the functional department of the Masheng People' s Hospital of Hubei Province for analysis. All patients were diagnosed by ultrasound-guided transrectal prostate biopsy. The diagnostic accuracy of transrectal prostate biopsy guided by ultrasound was analyzed according to the gold standard of pathological diagnosis. Results Of 102 patients, 54 (52.94%) were diagnosed as prostate cancer by puncture biopsy, and the remaining 48 (47.06%) were benign diseases. The diagnostic accuracy, sensitivity and specificity were 96.08%(98/102), 94.64%(53/56) and 97.83%(45/46) respectively. Some patients suffered from pain, hematuria, fever and other complications, and recovered naturally after a few days. Conclusion Ultrasound-guided transrectal prostate biopsy has high sensitivity and specificity in the diagnosis of prostate cancer. It can improve the accuracy of preoperative diagnosis of prostate cancer. Although there are some complications, the degree of complications is slight, and it can recover by itself. It has high clinical value.
