摘要
目的探讨糖化血红蛋白水平对糖尿病性心脏病合并心功能不全的影响。方法选取2016年1月~2017年12月我院收治入院的43例糖尿病性心脏病合并心功能不全患者作为研究对象,全部患者入院后给予糖化血红蛋白检查,根据结果将其分为两组,分为低糖化血红蛋白组(糖化血红蛋白≤6.5%)18例和高糖化血红蛋白组(糖化血红蛋白>6.5%)25例。同时,所有患者入院后均行空腹血糖、餐后2h血糖、三酰甘油、胆固醇、体重指数(BMI)等检查,并行心脏彩超检查、测量左心室射血分数(LVEF)。结果高糖化血红蛋白组的空腹血糖、餐后2h血糖分别为(7.50±2.36)mmol/L、(13.25±5.65)mmol/L,高于低糖化血红蛋白组的(5.53±1.12)mmol/L、(7.15±1.72)mmol/L,高糖化血红蛋白组的LVEF为(50.68±10.45)%,低于低糖化血红蛋白组的(61.34±11.33)%,差异有统计学意义(P<0.05)。两组患者的BMI、三酰甘油、胆固醇水平比较,差异无统计学意义(P>0.05)。结论通过对糖尿病性心脏病合并心功能不全患者实施糖化血红蛋白水平监测,可以对患者的近期血糖控制情况及糖尿病性心脏病合并心功能不全情况做出预判。
Objective To investigate the effect of glycosylated hemoglobin level on diabetic heart disease complicated with cardiac dysfunction.Methods A total of 43 cases of diabetic heart disease complicated with cardiac dysfunction patients hospitalized in our hospital from January 2016 to December 2017 were selected as subjects.After admission,all patients were examined for glycosylated hemoglobin after admission.According to the results,the patients were divided into two groups:18 cases in the low glycosylated hemoglobin group(glycosylated hemoglobin≤6.5%)and 25 cases in the high glycosylated hemoglobin group(glycosylated hemoglobin>6.5%).At the same time,the fasting blood glucose,2-hour postprandial blood glucose,triglycerides,cholesterol,body mass index(BMI)and other examinations after admission were examined in all patients,and left ventricular ejection fraction(LVEF)was measured by cardiac color dopplerultrasound.Results Fasting blood glucose and postprandial blood glucose in the high glycosylated hemoglobin group were(7.50±2.36)mmol/L and(13.25±5.65)mmol/L,which were higher than of the low glycosylated hemoglobin group with(5.53±1.12)mmol/L and(7.15±1.72)mmol/L,respectively,and the differences were statistically significant(P<0.05).The LVEF in the high glycosylated hemoglobin group were(50.68±10.45)%,lower than that of the low glycosylated hemoglobin group with(61.34±11.33)%,and the difference was statistically significant(P<0.05).There were no statistically significant differences in BMI,triglyceride and cholesterol levels between the two groups(P>0.05).Conclusion By monitoring the glycosylated hemoglobin level in patients with diabetic heart disease complicated with cardiac dysfunction,we can predict the recent blood glucose control and the condition of diabetic heart disease complicated with cardiac dysfunction.
